Purpose: Due to its anatomic and physiological features, ilecoceal is the part of high incidence of diseases mostly with ulcerative in nature. The application of electronic endoscopes and pathological biopsy to this area has greatly improved the diagnosis of such diseases. At present, endoscopes are used to perform the preliminary diagnosis, and biopsy incorporating with special tests of various kinds is used to finalize the diagnosis. As the clinic observations of ulcerative diseases in ilecoceal are very similar, doctors in this field are often found unable to come to an accurate diagnosis. Thus the incidence is very high regarding the lesion caused by delay in appropriate treatment or wrong treatment. It is in this spirit that the author of the present paper attempts to, based on the analysis of documented history from January of 1990 to December of 2003 concerning the cases of affection in ilecoceal,- A -probe into both the construction and proportion of the lesion of ilecoceal ulcerative diseases, analyze both its morphs and its morphs of biopsy under the endoscopes as well as the correct diagnoses and evaluate the diagnostic significance in terms of using endoscopes and biopsy. The studies aim at producing some novel methods for diagnosis of lesion caused by ulcerative diseases in ilecoceal. Method: (1) Collection of the history cases receiving endoscopes check from January of 1990 to December of 2003 at the Endoscopes Treatment Center of Nanfang Hospital; (2) diagnostic criteria: ulcerative ilecoceal under endoscopes and further verified by clinic behaviors and special checks incorporating with relevant operations or pathological confirmation; (3) focuses on ilecoceal ulcerative diseases such as intestinal tuberculosis, Crohn's diseases, ulcerative colitis, colorectal malignant lymphoma, colorectal cancer (ulcerative in nature) and other pathological diagnoses under endoscopes; (4) review of the pathological results of biopsy and immunized tissues. Results: (1) In recent years, the rate of diagnosed affection in ilecoceal is observed higher. This is believed to have something to do with the increasing use of mucosa staining. (2) There were 203 cases of lesion in ilecoceal (accounting for 60.6%) clinically identified under the endoscopes, 132 undiagnosed cases, 39.4%. Among these cases, the diagnostic rate of pathological changes of ulcerative in nature was the minimum, 34%, while that of the lesion of eminence in nature was the maximum, 82.1%. (3) The incidence of lesion of ulcerative in nature was the highest. What we have got is as follows: 42 cases of eminence disease, 20.7%; 37 cases of Crohn's disease, 18.2%; 29 cases of ulcerative colitis, 14.3%; 21 cases of colorectal malignant lymphoma, 10.3%; 21 ulcerative colorectal cancer, 10.3%; 12 cases of infective diarrhea, 5.9%; 11 cases of simple ulceration, 5.4%; 3 cases of malignant cell lesion, 11%; 7 cases of ulcerative submucosal tumor, 3.4%; 5 cases of intestinal type of Behcet's-5-disease, 2.5%; and etc. (4) The analysis of the morphs of ulcerative ilecoceal under endoscopes confirms us that ulceration caused by different lesion has its own characteristics. This can be described as follows: while the cases of simple ulceration accompanied by tumors are mostly colorectal cancer-oriented and the cases of spreading ulceration in the overall colon or colon in the right side observed as the continuous mucosa lesion are usually ulcerative colitis-prone, the cases of vertical ulceration with spring pathological changes accompanied by narrower cavity of intestines are generally Crohn's disease-proved. What should be mentioned in our research is that such pathological changes are, though, similar to malignant lymphoma, their morphs observed under endoscopes are not so obvious and are easily mistaken as other changes in pathological in nature. (5) The diagnostic rate of intestinal tuberculosis by pathological means of endoscopes and biopsy turns out to be 35.7, of Crohn's disease, 29.7%, of ulcerative colitis, 72.4%, of malignant lymphoma, 33.3% and of...
