Objective: To investigate the value of differential diagnosis in astrocytoma and metastatic carcinoma of brain by Magnetic Resonance Imaging Spectroscopy (1HMRS).Method: A series of 40 cases operated in our department was analysed respectively by Magnetic Resonance Imaging Spectroscopy. There were 20 cases of astrocytoma and 20 cases of metastatic carcinoma in brain. They were all scaned by MRI and MRS during their medical treatment process. The presenting feature, operative details and complications were documented.Results: Spectra of different areas were taken and fitted areas in the spectrum for NAA, Cho, Cr and other metabolite peaks were measured and Cr/NAA, Cho/NAA, Cho+Cr/NAA, Cho/Cr ratios were calculated for each vowel. These ratios can direct positive differential diagnosis between astrocytoma and metastatic carcinoma of brain .Conclusions: Multivoxel 1HMRS and enhanced MRI will be useful in the differential diagnosis between astrocytoma and metastatic carcinoma of brain. Cr/NAA, Cho/NAA, Cho+Cr/NAA, Cho/Cr ratios can indicate the differences of contents among N-acetylaspartate, choline, creatine, lactate, lipids and myo-inositol in nervous system tumors. The ratios had obvious statistics discrepancy. Therefore, Magnetic Resonance Imaging Spectroscopy can identify astrocytoma and metastatic carcinoma of brain as strain level.
