Part one Analysis the clinical data of dental trauma Objective: To analysis the dental traumatic data clinical from our department. Methods: The dental records of all patients presented for treatment at the teaching clinics of department of Endodontics from January 2003 to December 2003 were examined for the collection of data relating to age, gender, cause of injury, number of injured teeth, type of tooth, type of injury, the time elapsed between injury and seeking of dental care.Results: 7920 patients attended the teaching clinics of Department of Endodontics for dental care. Of all patients, 64 patients(119 teeth) had suffered traumatic dental injury, with prevalence of 0.81%. Male 64%, female 36%. The leading type of injury was complicated crown fracture involving enamel, dentin and exposure of the pulp(26.89%) followed by enamel and dentin fracture(17.65%) crown-root fracture, may or may not expose the pulp(15.13%). Combined with enamel fracture, the incidence of these four types of injury was 67.23%. The incidence of injuries to the periodontal tissues and root fracture was 23.47% and 8.30% respectively. Distribution of type of tooth. 78 maxillary central incisors(65.55%); 21 maxillary lateral incisors(17.65%); 6 maxillary canine(5.04%); 14 other teeth(11.76%). The leading cause of injury was falls (28.1%), followed by traffic accident (21.9%), sports (21.9%) and collisions (20.3%). Male had more injuries owing to sports ( 29.3% ), collisions (19.5% ), traffic accident (19.5%) and falls (19.5%); Female had more injuries owing to falls (43.5%) while only 8.7% owing to sports. Distribution of patients according to gender and age The proportion of patients among 16-35- year-olds who had sustained dental injury was 71.88%. Most male patients were among 16-35- year-olds while most female patients were below 25 (73.91%). Duration of time elapsed between the injury and the presentation for dental care. Only 23.08% (15/65) of the patients sought treatment the same day and 15.38% (10/65) theday after injury, 24.61% (16/65)had a treatment delay exceeding 1 month. Distribution ofpatients according to time elapsed and type of injury showed the trends that the more severe of the injury the earlier the patient came for treatment. Distribution of teeth according to type of injury and the number of teeth involved in one patient. 33 cases had more than one tooth involved and 23 had more than one type of injury (69.10%, 23/33). Tooth avulsion was always involved with other tooth. Enamel and dentin fracture, crown fracture with pulp exposed were also like to involved more than one tooth, the incidence of involving other tooth was 81.00%(17/21) and 78.10(25/32) respectively. Conclusions: 1. Male was easier to suffer dental trauma. 2. Maxillary incisors were the most often affected teeth. 3. Young people (16-35-year-olds) was easier to suffer dental trauma. Female among 16-25-year-olds and male among 16-35-year-olds were easier to suffer dental trauma. 4. Crown fracture (Enamel fracture, Enamel and dentin fracture with/without pulpal involvement, Crown-root fracture, may or may not expose the pulp) was most common by the type of injury. 5. Sport was the most common cause to male while falls, traffic accident, collision was the leading cause to female to cause dental trauma. 6. The treatment time elapsed was related to clinical symptom. The early treatment was due to acute symptom and delayed treatment was related to the complication of the dental trauma. 7. Dental trauma may happen to a single tooth or more. Avulsion was most like to accompany with other traumatic teeth. Crown fracture was also like to involve more than one tooth. 8. When more than one tooth involved, it more like to have more than one type of injury.
