Diabetic cutaneous ulcer is harmful to diabetic patients' health. diabetic acra gangrene anddiabeticfootarecommonintheallkindsofcutaneousulcers. Werealizethatthedamagedskinresuledfromweakness,stasisandtoxincausedbylongcourseof diabetes mellitus is the basic pathology of the diabetic skin ulcer , and suggest that the therapy isbased on the principle of YiQi HuoXue TongLuo(YQHXTL). The principle of YiQi HuoXueTongLuo is focus on YiQi TongYang at the same time emphasizing the theory of skin damaged byweakness,stasis and toxin, which expands the theorical methods of diabetic skin ulcer.In order todiscussing the pathogenesis of diabetic skin ulcer and the mechanism of the principle ofYQWYHXTL on the diabetic skin ulcer' healing, we observed the effect of the traditional Chinesemedicine on the proliferation of fibroblast(FB) in diabetic rat's skin wounds and the effect ofYangYu paint on the diabetic rat's skin wounds by creating the model of diabetic rat's skin wound ,and detected such changes as the proliferation of FB- the synthesis of ECM - MMPs TIMPs – thehealingofwoundbythemethodsofhistopathology immuohistochemistryandbitteracid-staining,sowefoundtheprocessofthediabeticskinwound'healingandtheresultsasfollowed:(1) The proliferation of FB could be improved when the concentration of astragalus polysaccharideswas 156ug/ml 39ug/ml 9.75ug/ml and of Japanese cinnamon bark aldehyde was 22.4ug/ml11.2ug/ml 5.6ug/mlandofligustrazinewas31ug/ml 7.75ug/ml 1.94ug/mlandofionmusconewas 2ug/ml 1ug/ml 0.5ug/ml. When the concentration of the group of YQWYHXTL was58.45ug/ml和29.225ug/ml, the actions of the proliferation and migration of FB were better thanothergroupsandtheactionoftheformerwasbetterthanthelater.(2)The healing time of the model group 27.13 1.81d was much later than the control group15.25 1.67d (P<0.001). Thewoundhealingrateinmodelgroupwassignificantlylessthanthecontrol group. The number of FB in the model group increased slowly, while the growth of FBachieved the peak at the 7th day in control group, then decreased slowly. The number of FB at thethird day ,the 7th day, the9th day, the11th day in the model group was significantly less than thecontrol group. The expression of PCNA and the number of FB showed the positive correlation inbothgroups.Theoddsoftipe collageninthemodelgroupwasmarkedlylessthanthecontrolgroup. The tipe collagen increased slowly at each times in the model group while increasedmarkedlyinthecontrolgroup.Tipe collageninbothgroupsdecreasedandthecontrolgroupwasmore markedly than the model group. The expression of TIMP-1 in the model group increasedslowly at the low level, which showed the positive correlation with the number of FB in the wound;and the expression of TIMP-1 in the control group achieved the peak at the 11th day , decreasingsignificantly after that , which showed the positive correlation with the number of FB in the wound.4 益气温阳活血通络法对糖尿病性皮肤溃疡愈合的作用机制研究The expression of MMP-1 in the wound of the model group increased slowly, achieving the peak atthe 11th day and decreasing markedly after that ; while MMP-1 in the wound of the model groupexpressed in a large amount at the third day , achieving the peak at the 7h day and decreasingmarkedly after that. The expression of MMP-1 in the model group was higher notably than thecontrolgroupatthe11th,15th,30thday.TheexpressionofMMP-13inthecontrolgroupachievedthepeak at the 7th day and decreased greatly after that; while the expression of it in the model groupincreasedslowlyandachievedthepeakatthe11thday,decreasingslowlyafterthat,andmuchhigherthanthecontrolgroupatthe11th,15thday.(3)The times of wound healing showed significantly difference(P<0.01) between the big dosagegroup(16.38 1.19d) themiddledosagegroup(17.5 1.2d) thesmalldosagegroup(21.38 1.3d)ofYangYu paint and the model group(27.13 1.81d).
