Objective:1 To investigate the feasibility and effect of applying chemical biliary duct embolization(CBDE) for chemical hepatectomy, equally carry though the filter of biliary duct embolic agent.2 To discuss the influence of CBDE on apoptosis and proliteration of hepatocyte.3 To probe in the influence of CBDE on collagen and its regulating cellular gene,also preliminarily analysis the mechanism of chemical hepatectomy.4 To detect the value of applying CBDE to prevent the post operative recurrence of hepatolithiasis.Method:1 Choose the combination of phenol and cyanonacrylate or the combination of absolute ethanol and cyanonacrylate as the embolic agents ,The influence of CBDE on liver weight and hepatic function was discussed ; The effect of chemical hepatectomy was also observed by the change of hepatic histology; Thus,the effect of two embolic combination was compared.2 The destroy effect of phenol and absolute ethanol on biliary mucosa was observed;The degree of biliary duct lumen was occluded;We also observed the ability of cyanonacrylate thatdispersing to the twig of biliary duct,in order to analysis weather the combination of sclerosant and adhesive agent can completely embolize the target biliary system.3 The infuence of CBDE on hepatocyte apoptosis and proliferation was discussed by Fas and PCNA; We also analysis the function of fibrogenic cellular gene-TGF-β1 .4 The mechanism of CBDE was preminilrily discussed by collagen, TGF-β1,TIMP1,Type Ⅰ,Ⅲ, Collagen.5 Using the animal model of hepatolithiasis, the effect of CBDE on PC and P-glucurodinase was investigated;The value that CBDE prevent the recurrent postoperative hepatolithiasis was also analysised.Results:1 CBDE can simultaneously oppress and obstruct the adjacent hepatic artery and portal vein,induce the hepatocyte to massively die out,replaced by proliferative biliary ductules and collagen; Embolized hepatic lobe was obviously athrophy and fibrosis. In its peripheral part, the hepatocyte did not exist, thus, the complete effect of chemical hepatectomy was attained.2 Phenol,abusolate ethanol can preferably destroy the mucosa of intrahepatic biliary duct, can arouse the occlusion of biliary duct cavity, also can occlude the small biliary duct; Cyanoacrylate can disperse to the twig of biliary duct;On the basic effect of sclerosant,the biliary duct can be occluded more completely by adhesive agent.3 As the embolic agent,the combination of phenol and cyanoacrylate has better effect than the combination of ethanol and cyanoacrylate.4 CBDE can induce the hepatocyte apoptosis and can indirectly inhibit the regeneration of hepatocyte.5 CBDE can induce the embolized lobe to massively express collagen I > III and TGF-Pi, TIMPj ,startup the process of chemical hepatectomy.6 CBDE can reduce the expression of endogenous (3-glucurodinase ,also can effectively eradicate PC.There is not pigment stone come in being in the cavity of embolized biliary duct.The embolized lobe was obviously athrophy and fibrosis. CBDE can availably prevent the recurrence of hepatolithiasis. Conclusion:1 CBDE is feasible and safe.The effect of complete chemical hepatectomy can be possibly achieved.2 CBDE can possibly prevent the production of hepatolithiasis, possiblely can attain the effect of chemical resecting the focus of hepatic segment.
