PURPOSEAcetazolamide(ACZ), a cerebrovascular dilator, has been used abroad to assess cerebral blood flow(CBF) reserve by brain SPECT. This noval method is known as " ACZ stress test" .In this study we planned: (1) to establish a routine procedure of the " ACZ test " , (2) to obtain the normative data (both SPECT images and semiquantitative analysis values) for CBF response to ACZ injection, (3) to enhance the sensitivity and accuracy of detecting ischemic encephalopathy, and (4) to give criteria of CBF reserve that may be useful clinically for managing patients, monitoring therapeutic effect and assessing prognosis.METHODSBrain 99mTc-ECD SPECT studies before and after ACZ administration (1g i.v. 20min post injection) were performed on 6 normal subjects (mean 51 years), 32 cases with TIA or RIND(mean 56 years) and 11 cases with minor stroke(mean 63 years).XCT was performed in all patients and MRI, TCD or DSA in some of them.Image analysis: The images of baseline and after ACZ injection must be normalized before comparative analysis. 1.
