Objective:Osteoarthritis of knee (KOA)is a common disease of middle- and old-aged people, of which incidence is on a steady increasing trend along with longer life span and increasing number of old people. KOA is an arthropathy characterized by hyperosteogeny due to degenerative articular cartilage. The basic pathological characteristics of KOA are the degenerative or vanishing tissue in the arthrodial cartilage, osteophytes in the joint margin and secondary changes in subcondral bone. The whole process involves the entire joint, including subcondral bone, ligaments, synovium as well as the muscles surrounding of the joint, which eventually leads to painand dysfunction of the joint.The pathogenesy of KOA is still not totally clear though some deep research have been done on it. The pain and swelling of the knee has a great impact on the quality of life to the patient suffering from KOA. However, to date there is no effective treatment to reverse perosteogeny or to control the progressive pathological changes. It is also the reason why there is no ideal treatment cause all the cure method is symptomatic treatment. KOA at early stage can be treated by Traditional Chinese medicine (TCM) more effectively, it has the advantages such as economic, less side effects and simultaneous treatment of etiology and symptoms.My advisor professor Fan Yueguang holds that KOA is primarily caused by the deficiency of liver and kidney and precipitated by sprain, strain and invasion by exogenous pathogens, the swelling and pain of the knee is the result of stagnation of Qi, blood and endogenous phlegm and damp as well as obstruction of meridians. Therefore, the treatment of KOA should be focused on the following principles: tonifying liver and kidney, promoting blood circulation so as to enforce the vital Qi and dispel the evil Qi and treat KOA from both the primary causes and symptoms.Based above theory, professor Fan focuses on the principle of tonifying kidney and promoting blood circulation in the treatment of KOA. He created the Formula of Guan jiekang to treat KOA. Long-term clinical follow-up indicated that the formula had a satisfactory therapeutic effect for KOA. Under the principle of evidence-based medicine, the study was designed with randomization and comparison. A systematic observation of the clinical therapeutic effect of Guan jiekang on the KOA was made in the research。Methods:This clinical study included 76 cases of KOA, aging from 40 to 70 years old, all of them came from the first affiliated hospital of Guangzhou University of TCM. The diagnosis was made according to the revision classified standard of American rheumatism association's osteoarthritis in 1995. The cases were randomly divided into two groups, Guan jiekang group and Puli collocystis (G1ucosamine) group. The patients in Guan jiekang group were given Guan jiekang and the patients in control group were prescribed Puli collocystis, the duration of both groups is 6 weeks. After the treatment, the therapeutic effect of the two groups was compared by the symptoms and signs (ache of the knee,function of the knee) of before and after treatment in the same group, furthermore, the contrast between the two groups was also performed to show the difference of the therapeutic effect of the two medications. Statistical method: numeration data is analysed with X~2-test, measurement data was analysed with t-test.Result: IN the two groups (Guan jiekang group and Pull collocystis group),the symptoms and signs after treatment were better than them before. And the contrast between the two groups was also performed to show great difference of the therapeutic effect of the two medications, Guan jiekang medications was surpass Puli collocystis medications.Conclusion: The Guan jiekang medications had a obviously therapeutic effect for KOA. Based on the result of this research, The Guan jiekang had better therapeutic effect than Glucosamine for KOA in six weeks.
