Interventional therapy guided by ultrasound is one of the most popular nonoperation. Because it is non-inasive, real-time, east to take and so on. However, for some deeply wounds of abdominal parenchymal organs, especially the need to pressure, it was difficult to treat. And requirements of hemostatasis are more. Given the characteristics and limitations of this technology, combined with our group pre-trauma of abdominal parenchymal organs of animal experiments and clinical analysis, it is considered that appropriate hemostatic agents should have good dispersion, mobility and the effective absorption. It will play an important role in treatment of trauma. In the preliminary test, the results showed that gelatin solution in wound treatment will plays an important role in hemostasis, and speculated that added some agents to crosslink, the solution will effectively reduce the bleeding time and closing the wound. In particular, any single component of the gelatin matrix hemostatic agent founded no obvious side effect and toxicity by a large number of reported. To confirm the hemostatics of injected gelatin matrix (HIGM) has good effect on hemostatasis and its mechanism, it was researched.Firstly, observed period of time the shape of gelatin (different molecular weights, different concentrations) on growth and compatibility of human umbilical vein endothelial cells (HUVECs), human hepatic cells (HH). And we also observed its effect on cell protection by MTT. The results showed that in pre-interference model, gelatin has a significant effect on cells. And higher molecular weight gelatin can increase cell growth. It may showed that gelatin within a certain range has good for repairing and reconstruction of microvasclar and hepatic cell. It may be one of the mechanisms of promoting tissue repair.Secondly, in liver trauma model by Wister rat, we observed that gelatin matrix hemostatic agent treated by the single enzyme in ultrasound-guided for the traumatic parts. Recorded bleeding time and to evaluated tissue repair drug absorption, we observed the sites by biopsy specimens at 3,6, and 9 day. The results showed that, for the diagnosis of liver injury, the contrast enhanced ultrasound (CEUS) (100%) have more advantages than general ultrasound (42.5%), Gelatin-based hemostatic agent single enzyme in ultrasound guidance, can be stop bleeding in a short time at liver trauma, especially in the case of non-pressurized. Besides, we found that gelatin-based hemostatic agent single enzyme can not only absorb in the short term, and can greatly reduce the probability of abdominal adhesions.Thirdly, In order to accelerate reduce the bleeding time (controlled in a minute or so), the proposed method of in vitro coagulation test tube to observe the HIGM the bleeding time, clot weight, and other related indicators. The results showed that HIGM's related indicators are better than other control groups, particularly in the shortest bleeding time and effectively promote the maximum clot formation.It was established the inferior vena cava (IVC) injury model by New Zealand rabbits. After treatment by ultrasound-guided, recorded the bleeding time, drug absorption and observed pathological tissue repair. The results showed that, HIGM can promote blood clot formation in a short time and effectively blocking blood vessels to injury. Observed for 15 minutes in thrombus stability, the movement does not appear. And through the all-digital color Doppler ultrasonic diagnostic flexible Qualitative and quantitative analysis of imaging the elastic HIGM thrombosis, suggesting HIGM thrombus more stable.Therefore, we believe that abdominal parenchymal organs for emergency trauma treatment, HIGM can be rapid hemostasis, the treatment process without pressure, and absorbed within a week and so on, it will likely be the most appropriate intervention to stop bleeding using drugs.
