Objective Chronic Obstructive Pulmonary Disease (COPD) remains a major public health problem.It is the fourth leading cause of chronic morbidity and mortality in the world, and is projected to rank fifth in 2020 in burden of disease caused worldwide, according to a study published by the World Bank/World Health Organization. Based on the suggestion of Global Initiative for COPD (GOLD), different Nations could make different programs focus on their social health sources. Traditonal Chinese medicine(TCM) is a special therapeutic way on management and prevention in our country and so many clinical researches show that TCM has positive effect on preventiving acute exacerbation and improving the manifestations in stable COPD. Since the system.for evaluating healing effect is not properly, to set up a good program for telling the treatment result is a major project.Methods:This research was a random control clinical trail with double blind method in multiple centers. We analysed the baseline dates to evaluate which index that we' ve chosen has good relationship with the TCM sydrome. The indexes contained TCM symptoms table, respiratory function examination,6MWD, SGRQ, immunology cells and inflammation factors.Results:In clinical research, we found the TCM symptoms only related with the conditions of exacerbation, SGRQ, smoking months and CD8+ T cells. The smoking months had line relationship with IL-8 and TGF-β1. According to the symptom of sore feeling on lion and knees, we divided patients into kidney deficiency syndrome group and lung deficiency syndrome group. All indexes in the group of kidney deficiency syndrome were much more severity than the other group, included AECOPD, symptoms of cough with sputum, dyspnea and pants, intolerance of cold, night sweating and SGRQ score, also the express of TGF-β1 increasing obviously in this syndrome.In the laboratory research, we could only detect out the expression of TGF-β1 in the patients' serum, but not IL-8 and TNF-α. Conclusion:The conditions of exacerbation, SGRQ, and immunology cells had intimated relationships with TCM sydrome. These indexes could evaluate the therapeutic effects in treatment with TCM. The TCM symptoms questionenair was not very properly for accessing TCM treatment effect, since it is lack of items about the influences on the patients'activity and life conditions due to COPD, so it should be replaced by a better one. The kidney deficient syndrome was worse than the other syndrome in stable COPD not only manifests on clinical symptoms but also cellular factor,TGF-β1. So these factors could be used as indexes in predicting the prognosis of COPD.
