| Although antibiotic, positive hemodynamics and oxygen delivery were used, the death rate of septic shock is still higher than 50%. The infection caused by gram-negative bacterial is the main reason for septic shock. It has been realized publicly that endotoxin (LPS) can cause immune reaction in septic shock. The study of prevention and cure investigation on diseases caused by endotoxin is one of the most active fields in international medical field during recent ten years.This thesis includes three parts of literature reviews: the present progress on septic shock and lung injury, recognition of TCM on septic shock and therapeutic progress, and pharmacological action of American ginseng and rhubarb.First, we advance a new thought that is combinating eliminating toxin with strengthening the vital-qi for treating septic shock. In TCM septic shock belongs to syncope and prostration syndrom, which is affected with virulent pathogen. When the vital-qi was consumed, together with flow of qi being reversed and primordial qi being exhausted, prostration syndrome will happen finally. The combination of toxin and pathogen is the basis of syncope and prostration syndrom, and exhaustion of yin and yang is the exorable trend during the development of illness. In the process of septic shock, the "outside toxin" includes microorganism, pathogen and toxin. Because "outside toxin" disturbed body's normal metabolism and function, a great quantity of free radical and cytokines are produced. These pathologic products belong to damp-heat and toxin which secondary to "outside toxin" can be called "endogenic toxin" together. "Outside toxin" together with "endogenic toxin" makes the toxin inside body more strong, primordial qi weaker, and finally causes exhaustion of yin and yang leading to death. The whole process presents the state of yin and yang reverse and combination of toxin and pathogen. So, we adopt combinating-eliminating toxin with strengthening the vital-qi for treating septic shock.Secondly, we discuss the prescription characterization of "FUZHENGPAIDU" injection. According to his clinical experiences and aiming directly at the complex pathological state of septic shock, professor Jiangliangduo suggests dispelling toxin together with strengthening body resistance for treating septic shock and prepared vein injection - "FZPD" injection (main drugs: American ginseng and rhubarb) for treating septic shock, which is syncope and prostration syndrom due to qi and yin exhaustion and pathogen and toxin flaring up in TCM theory. This prescription is different to Shengmai and Shenfu injection. It emphasizes both strengthening the vital-qi and body resistance as well as dispelling toxin, and embodies differentiation according to syndromes of TCM, coincidence to pathological mechanism of septic shock better. And we discussed problems in duplicating animal model in thesis.On the basis of endotoxin-induced animal model, we observed the effect of "FZPD" injection on rabbit endotoxin shock and lung injury. The experimental research includes five parts as following: 1 The rudiment study on the pharmacodynamics of FZPD injectionIn this study, when injected endotoxin in vein, rabbit showed shortness of breath, lip cyanosis, blood stasis in marginal ear vein rapid heart rate, even arrhythmia, low temperature and cold limbs. Blood pressure began to descent at 0.5h,and at 1h descents obviously, then drop down slowly, continuously. The endotoxin shock model was duplicated successfully.After treated with FZPD injection, the animal general state was improved. The blood pressure trend of was same with model group in general, but the time of blood pressure descending was later than model group, dropping obviously at 2h and the dropping degree was more lightly than model group. It showed that "FZPD" injection had good effects on improving model animal's general status, delaying the descent of Mean arterial pressure, so "FZPD" injection has distinct curative effects on endotoxin shock.2 The study on the effects of FZPD in...
