As the main types of stroke, Ischemia with high incidence, mortality rates and disability becomes extremely harmful disease to be tackled in the world. Therefore, research, prevention and treatment of ischemic stroke, is the important subject for basis and clinical medicine. Chinese medicine has a certain advantage in the prevention and treatment of ischemia, The Point Application of that plays a double role in the treatment through specific drug with stimulating specific points. But there had not been systemic Clinical and Experimental Research all the time. This subject was for the objective assessment of Clinical effects and the research for Experimental Methods and its mechanism with the therapy of Acupoint Application on ischemic stroke.Part I Literature Research1.Knowledge of ischemic stroke with Western MedicineAccording to the literature of both domestic and abroad, This article summarized the pathology and treating progress of ischemic stroke, the effects of Vascular Endotheliat Growth Factor (VEGF), superoxide dismutase (SOD), malondialdehyde (MDA) on the pathogenesis and treating evaluation of ischemic stroke.2. knowledge of ischemic stroke with Chinese medicineThe traditional medicine think that stroke should be understood as the encephalopathy in the brain, with the reaction in the viscera and meridians, the performance of a group of syndrome in the the viscera or meridians. According to the pathogenesis and place of the disease, it should adjust yin and yang, promote Qi and active blood for treatment in the brain. This article summarized the differentiation for syndrome classification, combination therapy of acupuncture and its mechanism.3. research progress of Point Application therapy on ischemic stroke The prospects of Acupoint Application therapy on treating ischemia are broad, its essence is a compound treatment of Meridian, Point and drugs. This article focused on the summary and review on Point , drug selection and the mechanism of efficacy.Part II Experimental ResearchObjective:To explore the mechanism of Point Application in improving ischemic stroke. Methods:A rat model of focal cerebral ischemia was made by MCAO of electric coagulation. The rats were randomly divided into 5 groups, concluding point application group, positive medicine control group, acupuncture group, nomal group and model group. The Removing-blood-stasis Sticker was used on Dazhui, Baihui and Mingmen point. The infarction volume was calculated by TTC tinctionm. The cerebral water content was measured with dry-wet weight method. The expression and protein level of VEFG was detected with immunohistochemical and enzyme linked immunosorbent assay method. Results:The infarction volume coincided with the dominative scope of MCAO of electric coagulation. The cerebral moisture was obviously different between each treating group and model group(P<0. 05). The expression and protein level of VEGF around infarction site of point application significantly increased as compared with that of model group(P<0. 01) and it was indiscrimate with that of Nimodipine group and acupuncture group(P>0.05). Conclusion:Point application of "Removing-blood-stasis" sticker can decrease hydrocephalus, promote the focal damaged cells to compose VEGF and accelerate forming of new blood vessels, resume blood stream providing of cerebral ischemic tissue. Positive control medicine—Nimodipine which is cerebrocrast can expand cerebral blood vessel and increase cerebral blood flow. Many experiments also proved that acupuncture can improve cerebral blood flow. The experimental results proved the protection to cerebral ischemia of "Removing-blood-stasis" sticker was similar with Nimodipine and acupuncture. As the further research, point application will become one of economical and effective therapeutic methods for ischemic stroke.Part III Clinical Research Objective:To observe clinical effect of acupuncture combined with point application on ischemic stroke and its influence on plasma superoxide dismutase(SOD), malondialdehyde(MDA). Methods:117 cases were randomly divided into acupuncture group(Group A, 40 cases), acupuncture combined with "Removing-blood-stasis Sticker" (Group B, 35 cases) and acupuncture with "Placebo Sticker" (Group C, 42 cases). One period of treatment was 15 days. Results:The effective and apparent rate of Group B was 91.43%,60% respectively, there was significant difference with Group A(P<0. 05). While there was not significant difference between Group C and Group A. Simultaneously, SOD content distinctively went up, MDA content distinctively went down in Group B, while no significant difference with Group A(P>0.05). Conclusion: The points used "Placebo sticker" were same with that of "Removing-blood-stasis sticker" , but there was no effective stimulation on them. It proved that "Removing-blood-stasis sticker" stimulated the points with absorption and metabolism of the medicine to develop point-meridian affection and pharmacological action. It also demonstrated that acupuncture combined with point application can make SOD content going up while MDA going down to an approximately normal level. This proved acupuncture combined with point application treat ischemic stroke with possible mechanism of eliminating the free radicals, while the other possible effective approach still need further work.
