Acute myocardium infarction(AMI)is a very serious diease,Being coronary occlusion and the broken off bloodstream,part of myocardium necrosis.Along with improving of living level,Acute myocardium infarction is one of the importent died reasons.Kaixin capsule is developed by the professor Chenjinghe,which has been identified with the clinical and experimental effects on symptomless myocardial ischemia,myocardial infarction ventricle remodeling,cardiac insufficiency after ischemic cardiomyopathy and cardio-pulmonary resuscitation.Kaixin capsule is mainly composed of yuejuwan, shixiaosan and shengmaisan,which has a function of yiqishengrnai,huoxuetongluo.The article aims to evaluate the effect of Kaixin capsule on the stimulating angiogenesis and reversing remodeling,explore the mechanism of Kaixin capsule on the Acute myocardium infarction patients who has the syndromes of qixuxueyu,also discuss the inference of reperfusion injury and left heart function.The article has three aspects, including literature investigation,experiment and clinical investigation.Literature studyThere are not terms of coronary heart disease in traditional Chinese medicine. According to the pathogenesis and clinical situations of Acute myocardium infarction,it looks like "Xiongbi,zhenxintong" described in traditional Chinese medicine.Meanwhile, the symptom,pathology,therapy,and featured traditional Chinese medicine of the ancient time will be introduced in the article,among which,some are still used and investigated in treating of Acute myocardium infarction nowadays.At last,there are some aspects about pathophysiology,treatment,and the mechanism of angiogenesis in acute myocardium infarction.experimentationexperimentation 1.The influence about Kaixin capsule to ET,NO and cholcsterol in experimental of acute myocardium infarction rats.After Sprague-Dawley rats were fed with hyper cholesterol feed for 4 weeks,the left anterior descending branch of coronary artery was ligated to establish Acute myocardium infarction model.Acute myocardium infarction rats were divided randomly into high dosage of Kaixin capsule group,low dosage of Kaixin capsule group,postive control group,AMI model group,pretend operation group.The results showed high dosage of Kaixin capsule obviously reduce the rats' ET content,which had a significant statistic sense in comparing with AMI model group(P<0.01).And high dosage of Kaixin capsule obviously improve the rats NO content,which had a significant statistic sense in comparing with AMI model group(P<0.01).High dosage of Kaixin capsule also obviously reduce the rats' cholcsterol,which had a significant statistic sense in comparing with AMI model group(P<0.01).It is pointed out that the function which Kaixin capsule protect the rats' myocardium maybe relate to improving NO and reducing ET and modifying the endocrine function of endothelium cell.Experimentation 2.Studying the change about density of myocardium micromodule decomposition and organization morphology of rats and researching the influence of VEGF and Flt-1 protein of myocardium organization in the experimental AMI rats with zymogram analysis method and immunohistochemical method.The results showed:high dosage of Kalxin capsule obviously reduce the rats myocardium infarction area,which had a significant statistic sense in comparing with AMI model group,And high dosage of Kaixin capsule obviously improve the ratsⅧcontent,which had a significant statistic sense in comparing with AMI model group(P<0.01).Observing the blood vessel of myocardium lateral branch,Kaixin capsule group obviously improve more density and number of micromodule than other groups.And high dosage of Kaixin capsule obviously improve the rats'VEGF and Flt-1content,which had a significant statistic sense in comparing with AMI model group(P<0.01),It is pointed out that Kaixin capsule has obviously function on promoting newborn myocardium blood vessel maybe relate to high inducting the VEGF and Flt-1 protein express myocardium.Clinical researchObjective:To explore the effect and mechanism of Kaixin capsule in AMI patients with the syndroms of qixuxueyu.Methods:48 cases of patients were selected,according to the criteria of both the syndrom of qixuxueyu.All patients were allocated randomly to two groups:Kaixin group and the control group.Both group were acceptd with thrombolytic therapy.Kalxin group were treated with Kaixin capsules at the same time. After 5 days of the treatment,the levels of cardiac muscle enzyme,NO,ET and the symptom incidence rate of the patients(angina,myocardium infarction enlarge,cardiac sudden death)were measured.After 3 week all the patients were checked with ultrasound cardiogram.E,A,E/A,EDV(The left ventricular end-diastolic diameter),DSV(The left ventricular end-systolic volume of the patients),LVEF(left ventricular ejection fraction) and the score of the syndroms of the patients were measured.Result:There were significant difference between Kaixin group and control group in clinical symptom total curative effect aspect,kaixin capsule can improve the syndrome of the qixuxueyu,such as chestpian, heart-throb atony and so on.kaixin capsule can improve the ischemic changes in ET and there were significant difference between two groups(P<0.05).And kaixin capsule can raise the level of NO and there were significant difference between two groups(P<0.05),kaixin capsure can reduce the cardiac muscle enzyme release and reduce the radio of arrhythmia. Also Kaixin capsure can raise the radio of E/A,LVEF and reduce EDV and DSV which had a significant statistic sense in comparing with control group(P<0.01).Conclusion:1.Kaixin capsule has a definite effect on AMI patients with the syndrom of qixuxueyu.2.kaixin capsule may have the ability of protect the cardiac muscle cell and relief reperfusion injury. 3.kaixin capsule can advance the left heart function.
