【Objective】1.To establish the differential protein express spectra: separate the protein of the cancer tissue and their adjacent normal lung tissue of NSCLC patients, establish the 2-dimensional gel electrophoresis(2-DE) atlas of differential TNM staging and pathological types of NSCLC, and identified the proteins by Matrix-Assisted Laser Desorption-Ionization Time-of-Flight Mass Spectrometry (MALDI-TOF-MS).2.To establish the proteomic prognostic evaluation of NSCLC, and to find the prognostic factors of the same TNM staging and pathological type: compare the 2-DE atlas of cancer tissue and the adjacent normal lung tissue, screen the differential proteins, and identified by MALDI-TOF mass spectrometry; then test this protein expression and analyze the survival time in the NSCLC patients who received surgery in our department, find the prognostic protein.【Methods】Analyze the proteins in 120 pairs of surgical specimens by the comparative proteomic techniques, including 2-DE and Image Master elite 3.10 software, and identified the protein ID by MALDI-TOF-MS. Retrospectively analyzed the protein expression and survival time in the105 patients who received surgical treatment, by immunohistochemistry test (Hercep test, DAKO kit) and COX risk and regression model, to find the prognostic protein.【Results】1.In the surgical specimens ofⅠstage squamous cancer, 9 proteins were up-regulated in the cancer tissue, including the MDM2 protein,heat shock protein 70(HSP70),HER-2 protein,heat shock protein 27(HSP27),galectin 1,nuclear FMRP interacting protein 1,calgranulin B,hemoglobin; and 6 proteins were down-regulated in the adjacent normal lung tissue, including ATP synthetase,neuronal specific transcription factor,microtubule protein,zinc finger protein,Triose-phosphate isomerase,crytallinα.2.The 6 groups (Ⅰ~Ⅲstage squamous cancer andⅠ~Ⅲstage adenocarcinoma) showed the same differential protein point, identified as HER-2 protein.3.The retrospective analysis showed that in the 105 patients of squamous cancer and adenocarcinoma, 53%(56/105) has the HER-2 overexpression, and 47%(49/105) has none-overexpression.【Conclusions】1.The expression level of HER-2 protein was an independent prognostic factor of theⅠ~Ⅲstage squamous cancer and the adenocarcinoma patients who received surgical treatment. The overexpression of HER-2 protein is related to poor prognosis. HER-2 protein could be the potential target of the NSCLC target therapy.2.Age, sex, pathological type were the factors that had no relationship with the prognosis of he patients ofⅠ~Ⅲstage squamous cancer and the adenocarcinoma who received surgical treatment.3.For the patients ofⅠ~Ⅲstage squamous cancer and the adenocarcinoma who received surgical treatment, the surgical operation plus adjuvant chemotherapy had better survival outcome than surgical treatment.
