Part OneThe Significance of Angiogenesis in Nasal inverted papilloma Malignant TransformationObjectives:To evaluate the clinical significance of angiogenesis in nasal inverted papilloma(NIP) malignant transformation.Methods:Pathologic frozen section of patients with NIP(23 cases),squmamous cell carcinoma(SCC) derived from NIP(8 cases) were studied.Serial sections were stained with CD105 by immunofluorescence and it's expressions were investigated. Microvessel density highlighted by CD105 were counted according to a standard protocol microvessel number of per unit area(MVNA).9 normal mucosa were used as control group.Results:CD105 was highly expressed in newborn microvessel,decreased in mature small vessels and middle vessels with smoth muscle.CD105 expression in SCC was significantly higher than that in NIP and normal mucosa(P<0.05).The mean MVNA value of SCC was 171.86±49.37,while in NIP was 123.35±26.02, and 33.91±17.38 in normal mucosa.Conclusions:Angiogenesis may play a significant role in NIP malignant transformation.CD105 is a reliable index in detecting newborn microvessls.The CD105-MVNA may be an important indicator to evaluate NIP malignant transformation. Part TwoThe Significance of TFPI-2 and VEGF-A165 in Nasal inverted papilloma Malignant TransformationObjectives:To localize the TFPI-2 and VEGF-A165 in NIP and it's associated SCC. To evaluate the significance of TFPI-2 and VEGF-A165 in angiogenesis and NIP malignant transformation.Methods:Pathologic frozen section of patients with NIP(23 cases),squmamous cell carcinoma(SCC) derived from NIP(8 cases) were studied.Serial sections were respectively stained with TFPI-2 and VEGF-A165 by immunofluorescence and observed by confocal laser scanning microscopy.9 normal mucosa were used as control group.Results:In control group,TFPI-2 expressed in mucosa epithelium,extracellular matrix,vascular endothelial cell and VEGF-A165 expressed in mucosa epithelium, mucus-producing goblet,vascular endothelial cell.In NIP and NIP associated SCC, TFPI-2 expression decreased,only detected in tumor cells and extracellular matrix. While VEGF-A165 expression upgraded in tumor cells.Conclusions:TFPI-2 expression decreased in NIP,especially in NIP associated SCC.VEGF-A165 expression upgraded in NIP and NIP associated SCC.TFPI-2 and VEGF-A165 play a great role in angiodenesis and may be important indicators to evaluate the malignant transformation reversal. Part ThreeThe Quantificated Expression of TFPI-2 and VEGF-A165 Gene in Nasal inverted papilloma Malignant TransformationObjectives:To quantificate the TFPI-2 and VEGF-A165 expression in NIP and it's associated SCC.To explore the significance of TFPI-2 and VEGF-A165 in NIP malignant transformation.Methods:Pathologic frozen samples of patients with NIP(23 cases),squmamous cell carcinoma(SCC) derived from NIP(8 cases) were studied.Western Blot and real time polymerase chain reaction(RT-PCR) were used to mensurate the gene expression of TFPI-2 and VEGF-A165.The correlation between TFPI-2,VEGF-A165 and MVD stained by CD105,TFPI-2 and VEGF-A165 were studied.9 normal mucosa were used as control group.Results:TFPI-2 expression in control group,NIP,NIP associated SCC were 109327.8±7024.3,103345.0±7931.4,93383.6±5044.1(Density INT/mm2,By Western Blot) and 1.45±0.16,1.51±0.18,1.61±0.15(By RT-PCR).The difference expression of TFPI-2 between NIP malignancy group and NIP group,control group were significant by statistics(P<0.05).VEGF-A165 expression in control group,NIP,NIP associated SCC were 94927.9±8633.5,110653.2±8419.7,116357.1±3926.5(Density INT/mm2,By Western Blot) and 1.26±0.07,1.21±0.08,1.15±0.09(By RT-PCR).The difference expression of VEGF-A165 in three groups were significant by statistics(P<0.05).VEGF-A165 has a significant positive correlation with MVD stained by CD105, r=0.735.TFPI-2 has a moderate negative correlation with MVD,r=-0.381.TFPI-2 has a undistinct correlation with VEGF-A165.r=-0.155(protein) and r=-0.110(mRNA).Conclusions:VEGF-A165 significantly correlated with MVD stained by CD105, while TFPI-2 has a moderate negative correlation with MVD.TFPI-2 and VEGF-A165 plays a significant role in NIP malignant transformation.The decreased expression of TFPI-2,which leads to weakness of inhibition to VEGF-A165,may at least take part in NIP malignant transformation.
