Part I:Study on the MR Spectroscopy Metabolic Characteristics of Prostate Cancer and Benign Prostate HyperplasiaObjective To quantify and compare the metabolic characteristics of normal prostate, prostatecancer(PCa),and benign prostate hyperplasia (BPH) by usingMR spectroscopy (MRS).Methods Twenty-one cases of Pca,23cases of BPH proved by operation or systemic biopsy,and17cases of normal prostate were examined by MRS.The prostate was divided into6regions (left/right bottom,middle,and tip), and the (Choline+Creatine)/Citrate (CC/C) value of each region was measured.After biopsy,all the puncture locations were marked and enrolled in one of the regions mentioned above.The average CC/C ratios of the normal prostate peripheral zone, the area of Pca, and the central zone of BPH were calculated.Results The average ratio of CC/C for prostate cancer(2.13±0.82) was statistically higher than that of normal prostate tissue (0.42±0.19) and the regions of BPH (0.62±0119)(t=0.725, P=0.000; t=0.684, P=0.000).Conclusion The difference of metabolic levels measured by MRS between PCa and BPH is statistically significant.MRS may beuseful in the differential diagnosis of PCa and BPH. Part Ⅱ:Study on the characterristics of three-dimensional magnetic resonance spectroscopy of prostatic intraepithelial neoplasia(PIN)Objective:To investigate and evaluate the imaging features in three-dimensional magnetic resonance spectroscopy in Prostatic Intraepithelial Neoplasia(PIN); Method:A total of38patients with higher level of prostate specific antigen (PSA) suspected prostate cancer were examined by MRI+MRSI, and then all proved by pathology of biopsy under TRUS,besides,collected another20patients in the same period benign prostatic hyperplasia MRI and MRS data,analysis the data of prostate lesions, conventional MRI characteristics, MRS test results retrospectivly, calculate (choline+creatine)/citrate,CC/C.Using repeated analysis of variance measures to compare the difference between benign prostatic hyperplasia group(BPH),prostate cancer group(PCa),low grade PIN group(LG PIN) and high grade PIN group(HG PIN).Results:The CC/C value of20BPH cases is0.62±0.21,0.76±0.22of7LG PIN cases,no significant difference between the two groups;The CC/C value of11HG PIN cases is1.72±0.49,3.49±1.64of20Pca cases.The CC/C value of HG PIN is statistically significant differente from other groups.Conclusion:The MRSI performance of LG PIN is similar to benign prostatic hyperplasia,however,the MRS changes of HG PIN is even close to prostate cancer.
