Psychological Stress Of Different Ethnic Population In Guangxi Bai Ku Yao Region And Its Association With Social Factor And Genetic Factor

Objectives (1) In order to understand the status of population in the Guangxi Bai Ku Yao area in psychological stress and related behavior.(2) In order to investigate the association of gene COMT, NPSR1, HSP90B1, HSP90AA2 and FKBP5 with the people in three national regions (the Guangxi Bai Ku Yao Zhuang, Han, Bai Ku Yao) on psychological stress and psychological characteristics.(3) In order to explore the interactions of genetic factors and social (environment) factors on psychological stress.Methods(1) The method of stratified random cluster sampling was used,193 Baiku Yao,200 Zhuang and 186 Han of a total 579 people over the age of 18 of each village as the research subject were randomly selected from 2 villages in Lihu and Baxu towns that Bai Ku Yao people lived,2 villages in Xichang and Guanshang towns that Zhuang and Han people lived together in Nandan county of Guangxi.(2)The self-designed questionnaires were used to survey the characteristics of demography and the factors of psychological stress related with behavior.(3) The height, weight, waist, hip circumference, waist circumference, blood pressure, heart rate et al were measured and the level of blood TC, TG, HDLC and LDLC were tested.(4) The life events scale (LES), simplified coping styles questionnaire (SCSQ) and social support rating scale (SSRS) were used to investigated the characteristics of psychological stress and psychology.(5) The SNP genotyping of gene COMT(rs6267、rs769224. rs4680), NPSR1 (rs12673132、rs6947841、rs324981、rs6972158), HSP90B1 (rs17034977), HSP90AA2 (rs2726836) and FKBP5 (rs1360780、rs3800373) were detected respectively by the method of iMLDR.(6) The data was analyzed with SPSS19.0 software and Amos5.0 software. The structural equation model was constructed to analy the relationship between psychological factors and psychological stress. The distributions of different demography were compared on the environmental factors and the gene factors. The methods of univariate, multivariate and logistic regression were used to analyzed the association of psychological stress with environmental factors and genetic polymorphisms.Results(1) The general life events scale scores in the fifty percentile (P50%) was 27 points and the scores of 293 subjects were more than 20 points with positive rate 50.60%. Among them, family events, the occurrence frequency of work and social events were 75.82%,11.57% and 24.35% respectively. The top three of life events were family economic difficulties in 234 people (40.41%), followed by debt due in 133 people (22.97%), and the third was housing shortage in 96 people (16.58%). The primary event that caused psychological stress was family events.(2) The score of general psychological symptoms was 44.39±13.16 ranged from 29 to 110. The factor scores of 80 subjects were over or equal 2 with positive rate 13.82%. The four factor scores of somatization, anxiety, depression and hostile were 1.75±0.63,1.47±0.58,1.51±0.58, and 1.32±0.45 respectively. The positive rate of psychological factor in the three ethnic groups varied from 8% to 34.48%. The anxiety factor was significant difference in the three ethnic groups. Comparing with the national norm, somatization factors of the three ethnic groups were higher than the norm, anxiety factor of Bai Ku Yao was higher than the national norm but hostility factors of Bai Ku Yao and Zhuang were lower than that of the national norm.(3) The overall coping style score 40.93±10.95, in which actively respond was 25.61±8.10 and negative response was 15.32±4.39. The highest of actively respond was Han, followed by Zhuang and Bai Ku Yao. The coping styles of negative or positive in the three ethnic groups were higher than the national norm.(4) The overall social support score was 27.72±6.07, in which the subjective support was 12.57±3.09, the objective support was 8.24±2.60 and the utilization of support was 6.91±2.42. The subjective support of Han was higher than that of Zhuang. Comparing with the Zhuang norm, the social support, subjective support and utilization degree of social support in the three ethnic groups were lower than that of norm, but objective supports were higher than that of the norm.(5) The distributions of genotype were examined in 11 alleles of 5 gene loci., The G/A frequency of rs 12673132 and C/T frequency of rs6947841 in NPSR1 were statistically significant difference between male group and female group (P<0.05). Comparing the data of different ethnic groups, there were significant difference (P<0.05) except the genotype and allele of NPSRl rs324981, HSP90B1 rs17034977 and HSP90AA2 rs2726836.(6) The total score of life events over or equal to 20 was designed as psychological stress positive, then frequencies of genotype and alleles in the positive groups were compared with the negative groups. The frequencies of genotype and alleles in Han and Bai Ku Yao were no differences between the two groups, while genotypes of COMT rs4680 and FKBP5 rs1360780, genotypes and alleles of NPSRl rs324981, were significant difference in Zhuang between the two groups (P<0.05).(7) The score of psychological symptom factors over or equal to 2 was designed as positive, then frequencies of genotype and alleles in the positive groups were compared with the negative groups. There had significant difference in FKBP5 rs1360780 and FKBP5 rs3800373 between the tow groups on anxiety factor in Bai Ku Yao(P<0.05). As for Zhuang, there had significant difference in genotype of COMTrs769224, NPSRl rs6947841, and NPSRlrs6972158 between the tow groups on anxiety factor (P<0.05), moreover it also had had significant difference in alleles of FKBP5 rs1360780 between the two groups on depression factor (P<0.05). As for HAN, there had significant difference in alleles of HSP90AA2 rs2726836 between the two groups on somatization factor. As for three ethnic groups, there had significant significance in alleles of NPSR1 rs324981 between the two groups on depression factor (P<0.05) and there had significant difference in genotype or alleles of COMTrs769224 and in alleles of HSP90AA2 rs2726836 between the two groups on hostility factor (P< 0.05).(8) There was significant difference in the distribution of NPSR1 rs6972158 allele in Han coping style (P< 0.05). The subjective support, objective support and support utilization were analyzed by multivariate analysis of variance model in Bai Ku Yao, Zhuang and Han, there were no significant differences among the three ethnic groups (P> 0.05).(9)The association between different genotypes and psychological stress of all subjects were analyzed by Logistic regression analysis of single factor (Logistic positive incidence of life events), the results showed that risk of carrying TT genotype had increased 0.445 times (95%CI:0.234-0.844) to psychological stress, if AA genotype of NPSR1 rs324981 was designed as the control. The difference was statistically significant (P<0.05).(10) Multivariate Logistic regression analysis showed that the scores of somatization, anxiety and depression increased risks of psychological stress by 2.381 times (95%CI:1.554-3.649),2.017 times (95%CI:1.148-3.544) and 1.845 times (95%CI:1.053-3.230) respectively, and the differences were statistically significant (P<0.05). Diastolic blood pressure and systolic blood pressure increased risks of psychological stress 0.981 times (95%CI:0.968-0.995) and 1.025 times (95%CI:1.003-1.048) respectively, and the differences were statistically significant (P<0.05).Conclusions(1) The positive rate of life events is higher in the Guangxi Bai Ku Yao population, in which life family events is primary cause for the psychological stress.(2) The total positive rate of psychological symptoms is in the normal range and the anxiety factor has difference between the three groups. The somatization factors of three ethnic groups and Bai Ku Yao anxiety are higher than the norm, but hostility factors of other two ethnic were lower than the norm. The negative coping, positive coping of three ethnic groups are higher than the national norm, in which Han is the highest in actively respond. The social supports, subjective supports and utilization degree of social supports in three ethnic groups, are less than Zhuang norm. However the objective supports are higher than that of the norm.(3) Gender differences exist in genotype or allele of NPSR1 gene rsl2673132 and rs6947841. There are difference in genotypes or alleles of COMT gene rs6267, rs769224 and rs4680, NPSR1 genes rsl2673132, rs324981 and rs6972158, FKBP5 gene rs 1360780 and rs3800373 among different ethnic groups.(4) In comparison the association of life events, coping style and mental symptoms with gene polymorphism among different ethnic group, there are difference in genes FKBP5 rs1360780 and rs3800373, COMT rs4680 and rs769224, NPSR1 rs324981 and rs6947841, NPSRlrs6972158 and HSP90AA2 rs2726836 respectively.(5) The genotype with T allele in NPSR1 rs324981 may be a protective genotype to psychological stress. Somatization, anxiety and depression and elevation of systolic blood pressure are risk factors to psychological stress, whereas low diastolic blood pressure is the protective factors to psychological stress.
