After China’s reform and opening-up, the industrialization and urbanization develop rapidly. With the loosening of generalized household registration system, which limited the flow of rural population to urban, rural labor force in the form of farmer workers could transfer to the secondary and tertiary industries in cities. The transfer of migrant workers to urban areas, not only promote economic development by providing cheap labor for urban sector, but also sitimulate economic growth in rural areas through the returing home to construct of migrant workers or through other means. With the growing population of farmer workers, problems about them, including medical secutity of them have become more and more important at nowadays stage.This thesis is just focous on the issue of medical security system for farmer worker under the macro perspective. There are seven chapters, divided into four parts in this thesis. The third and fourth parts are the main of the full-text analysis.The first part including Chapter I and Chapter II, is the groundwork and foundation of the whole framework. Chapter I, explicates the beckground, the motive, the logical structure, the methodology, the innovation and the shortcomings of the research. Chapter II focus on the researches and literatures about the production of farmer workers in economic growth, the demand and supply for farmer workers’medical security system.The second part consists of chapter III and Chapter IV, from the view of the medical security needs of farmer workers. The third chapter is about the generation of farmer workers, which are produced in the four conditions:the urban-rural dual structure, the manifestation of rural surplus labor force, the expansion of urban-rural gap, the absorption in the urban sector. The fourth chapter, on the basis of the third chapter, researches the health and insurance situation of farmer workers. Firstly, I evaluate the true latent health to calculate and decompose the health Gini coefficient, in order to find out the health inequality between urban and rural in China and between the farmer workers and citizens in citis. Secondly, I measure insurance inequality between the same people through the same methord with health inequality in previous step. Finally, I analyse the specific situation of migrant workers in illness and medical service utilization, to study desease risk of this group.The third part is Chapter V and Chapter VI, from the perspective of the medical security supply to farmer workers. Chapter V is about the international and domestic institutional environment for medical security system of farmer workers. Firstly, the health care systems in different countries or regions are described, to analyse the difference among five kinds of medical security systems, to find out the characteristics of structure of the system in a country or a region, and to make clear the role of the government in different system. Then the China’s medical security system for migrant workers is stuyed, in the dual-structure of China’s urban and rural medical insurance system, and in the construcition and reform process of the basic medical insurance system in urban.Chapter VI is pattern analysis about medical security system for migrant workers. In this chapter, all the different systems in different provinces and cities in China are divided into four patterns. And then on the base of the four patterns, the specific arrangements in Beijing’s, Shenzhen’s and Shanghai’s medical insurance for farmer worker are compared. Lastly, the feasibility of medical security system of farmer workers is researched, from the payment responsibility of the government, enterprises and migrant workers.The last Part is Chapter VII, in which the construction and development of the medical security system for migrant workers are studied. In the internal construction of the mecical security system for migrant workers, multi-level medical system, including medical aid, basic medical insurance and commercial insurance, should be built up according to different income levels and mobility of migrant workers. And a special medical insurance, which is national, is designed for those migrant workers with high mobility. In the external optimization of the medical security system for migrant workers, three medium-term strategies, including the regulation of the labor market, the training system for migrant workers and the optimization of urban construction, as well as a long-term strategy-to push forword the reform of China’s household registration system are discussed.
