Hip fractures are one of the most prone to fractures in the elderly. Many studies have shown that the mortality and the mortality risk of elderly patients with hip fractures are significantly higher than those of the same age group at home and abroad. A variety of organ dysfunction caused by excessive systemic inflammatory response in elderly hip fractures is an independent risk factor leading to death events. At present, there are some scoring systems and a variety of biomarkers in clinical, with a view to more accurate assessing patients condition and high risk level, reducing the degree of organ damage,improving the prognosis of patients. However, there are still some shortcomings in the diagnosis and treatment of multiple organ failure caused by systemic inflammatory response need to be further improved. In particular, it is necessary to find a more reliable,specific and sensitive biomarker for Disease monitoring and treatment evaluation of the disease in this area. With the deeply studing of the circular RNA system,find that it has a strong innovation and practical significance in this field,and provides new ideas and directions for clinical diagnosis and treatment.Part I: The differential expression of circular RNA in elderly patients with hip fractureObjective:The differential expression of circular RNA in peripheral blood of patients with hip fracture was analyzed by ring RNA microarray. It might find out some biomarkers and their regulatory mechanisms.The effective biomarkers and their regulatory mechanisms were provided for the early stage of hip fracture and prognosis.Method:Three subjects with normal hip fracture (test group) and 3 healthy subjects (control group)were selected. Blood samples were collected from the peripheral venous blood of the subjects. Plasma was extracted and the total RNA was extracted. The circular RNA was detected by circulating RNA in elderly patients with hip fracture. The bioinformatics method was used to predict the differences Expression of circular RNA related to miRNAs.Results:According to the different expression levels of circRNA between test group and control group ,the two groups of folding magnification (Fold Change, FC)> 2.0 or <-2.0, P<0.05 significant difference in the expression of circRNA data. It find out 2169 kinds of circRNA significantly different expression in the test group or in the control group,including 1173 kinds of up-regulated and there are 213 species different more than 5 times,996 kinds down-regulated and the different more than 5 times 151 species. The results of bioinformatics show that circular RNA can participate in the regulation of the occurrence and development of major complications in the early stage of hip fracture with miRNA.Conclusion:The expression of intracranial RNA in peripheral blood of patients with hip fracture was higher than that of the healthy elderly. Significant difference and specific expression of circular RNA were expected to be a potential diagnostic marker for the death of hip fracture in the elderly. The circular RNA expressed by the bioinformatics statistics could help studying the early aduance and the prognosis of the patients with hip fracture, provide a useful reference and supplement for the corresponding mechanisms studying.Part ?: The differential expression of circular RNA in elderly patients with hip fractureObjective:Using circRNA microarray technology to explore the differential expression circRNA in peripheral blood in the mortality in elderly patients with hip fracture which lay the foundation theory for the further studies on novel non-invasive biomarkers and the investigation of pathogenesis of the mortality in elderly patients with hip fracture.Method:The study has 30 cases of elderly patients with hip fracture, including 12 males and 18 females.One year after fracture, 3 cases died of pulmonary infection. The three death cases (death group) and othor three cases with ordinary hip fracture of the hip (control group) were collected to draw peripheral blood 4-5ml, plasma was separated, extract total RNA.Then circRNA Chip techrilogy was used to screen the differentially express of circRNA in the plasma of the elderly patients with hip fracture. The bioinformatics method was used to predict the differentially expressed miRNAs of circular RNAs. Finally,Real-time quantitative PCR was performed.Results:According to the different expression levels of circRNA between Death group and control group ,the two groups of folding magnification (Fold Change, FC)> 2.0 or <-2.0, P<0.05 significant difference in the expression of circRNA data. It find out 1263 kinds of circRNA significantly different expression in the death group or in the control group,including 670 kinds of up-regulated and there are 131 species different more than 5 times,593 kinds down-regulated and the different more than 5 times 145 species.hsa-circRNA-103637? hsa-circRNA-092417 two type of circular RNA express up trend by real-time quantitative PCR; three additional circular RNA such as RNA hsa-circRNA-100035? hsa-circRNA-045308? hsa-circRNA-001057express down trend by quantitative real-time PCR. The results of bioinformatics show that circular RNA can participate in the regulation of the occurrence and development of major complications due to death interact with miRNA.Conclusion:The expression of intracranial RNA in peripheral blood of patients with death after hip fracture was significantly higher than that of patients with common fractures. Significant difference and specific expression of circular RNA were expected to be a potential diagnostic marker for the death of hip fracture in the elderly. In addition, bioinformatics statistical differences in the expression of circular RNA can help studying mechanism of the elderly death patients with hip fracture, provide a useful reference and supplement for the regulary mechanisms studying.
