Background:Traditionally Deqi was considered crucial for acupuncture therapy,its importance is disputable in recent clinical studies.Reviewing the literatures of the research of the relationship between Deqi and effect,we found that the criterions of how to access Deqi were differently,and there was no method to judge Deqi.As a result,the results can not show the real effect of Deqi.Therefore,how to study the qualitative and quantitative judgment on Deqi in order to make the research results more reliable,whether there has the method to observe the phenomenon caused by Deqi objectively,stripping the subjective factors,to explore the characteristics of the effect of Deqi,further to the objective and quantitative Deqi,which make the result more reliable.All above are the current need to study and solve.Objectives:(1)Study on the effect of Deqi on acupoint surface temperature:according to the randomized design of modem evidence-based medical research,selecting primary dysmenorrhea patients with cold and dampness stagnation as the research object,to explore the influence of Deqi on surface acupoint acupunctures,and discussion on the feasibility of using the temperature change characteristics of acupoints as an objective index for evaluating Deqi.(2)Study on the relationship between the intensity of Deqi and the acupoints surface temperature:according to clinical evaluation scale of the subjects' sensations of Acupuncture,re-analyze the data of Deqi intensity and the acupoits surface temperature to explore the relationship between the Deqi intensity and the acupoits surface temperatureMethods:(1)Study on the effect of Deqi on acupoint surface temperature:total 68 patients with primary dysmenorrhea in cold and dampness stagnation pattern were included into our study and randomized divide into expected deqi(ED)group and expected non-deqi(END)group with a ratio of 1:3,which the cases were 17 and 51 respectively.Participants with menstrual cramps(VAS-P score equal to or above 40 mm)received acupuncture intervention on the first day of menstruation.Patients of ED group were acupunctured at SP6 by thick needle,deep insertion and some manipulation to achieve deqi.Contrary,patients of END group were acupunctured at SP6 by thin needle,superficial insertion and no manipulation to avoid deqi.All the patients were treated for 30 minutes.We measured the acupionts of SP6,SP8,SP9,SP10,RN4 and uterine surface projection area at 7 time points(before needling,needling 10min,needling 20min,removing needle,after needling 10min,after needling 20min,after needling 30min).Then,clinical evaluation scale of the subjects'sensations of Acupuncture developed by our team,was used to judge the real deqi conditions.According to the results,the patients were redivided into real deqi of ED group(A),real non-deqi of ED group(B),real deqi of END group(C)and real non-deqi of END group(D).Then to compare the acupoints surface temperature in each group at different time points.(2)Study on the relationship between the intensity of Deqi and the acupoints surface temperature:this study was involved the patients who really experiencing Deqi,according the intencity of Deqi to divide each group into three sub-group as weak Deqi intensity group,middle Deqi intensity group and strong Deqi intensity group.To compare acupoints surface temperature(SP6,SP8,SP9,SP10)of the different intensities of Deqi at 3 time points(before needling,removing needle and after needling 30min)in each real Deqi group.And then,comparing the acupoits surface temperature at 3 time points when it had equal Deqi intensity but with different needling methods.Results:(1)Study on the effect of Deqi on acupoint surface temperature:? Within group comparison:comparison of same acupoint surface temperature at different time points:there were significant acupoints surface temperature rising in right SP9 and bilateral SP10 of group ED(P<0.05),bilateral SP10 of group A(P<0.05),bilateral SP9 and SP10 of group END,group C and group A+C,right SP9 and bilateral SP10 of group overall the 7 time points(P<0.05),and there were significant acupoints surface temperature reducing in left SP8 of group D(P<0.05);comparison of bilateral same acupoints temperature at same time point:there were no significant differences.? Comparison between groups:there were no significant differences between group ED and group END,group A and group D(P>0.05),there were significant differences at all acupoints between group C and group D(P<0.05),there were significant differences at all right acupoints between group A+C and group D(P<0.05);there were significant differences at left SP6,SP8,SP10 and uterine surface projection area between group A and group C(P<0.05).(2)Study on the relationship between the intensity of Deqi and the acupoints surface temperature:?within group comparison:group A:comparing with middle Deqi intensity and strong Deqi intensity of the right,there were significant differences at SP8,SP9 and SP10 at before needling time point,there were significant differences at all acupoints at removing needle time point,there were significant differences at SP6 and SP10 at after needling 30min time point(P<0.05);group C:there were no significant differences(P>0.05);group A+C:comparing with weak Deqi intensity and strong Deqi intensity of the left,there were significant differences at SP6,SP9 and SP10 at before needling time point(P<0.05),comparing with weak Deqi intensity and strong Deqi intensity of the right,there were significant differences at SP10 at after needling 30min time point(P<0.05),comparing with middle Deqi intensity and strong Deqi intensity of the right,there were significant differences at SP10 at after needling 30min time point(P<0.05),there were significant differences at SP9 and SP10 at removing needle time point(P<0.05),there were significant differences at SP6,SP9,SP10 at after needling 30min time point(P<0.05);?Comparison between groups:comparing with group A and group C at middle Deqi intensity,there were significant differences of SP6 and SP8 at before needling time point and removing needle time point(P<0.05),there were significant differences of SP6 at after needling 30min time point(P<0.05).Conclusions:(1)Study on the effect of Deqi on acupoint surface temperature:?Needling at SP6 could cause 2 more acupoints surface temperature rising along spleen meridian;?The Deqi can cause the phenomenon of acupoints surface temperature rising through the spleen meridian SP9,SP10,RN4 and uterine surface projection area,No Deqi only can cause the phenomenon of acupoints surface temperature rising through the spleen meridian SP10;?The phenomenon appeared on Deqi and No Deqi were different,Deqi surface temperature rising can last all the treatment,but No Deqi only happened at the first 10min;?No Deqi was more easy to reduce the temperature of SP6 and SP8 than Deqi;?It can be concluded that the changes of body temperature can be used as one of the objective indexes to judge whether the Deqi is getting or not.(2)Study on the relationship between the intensity of Deqi and the acupoints surface temperature:?The strength of the Deqi intensity may be related to the temperature of acupoints surface;?The difference intensity of Deqi induced by acupuncture may be related to the surface temperature of acupoints and its location;?If there is a difference in the surface temperature of acupoints,the acupuncture stimulation may be different if wanna to induce the same Deqi intensity.
