Clinical Study On The Therapeutic Effect Of Touguling Rubber Ointment On Osteoarthritis

ObjectiveThe object of this paper is to evaluate the clinical efficacy and safety of Touguling Rubber Ointment in the treatment of osteoarthritis by objective Evidence,so as to provide clinical basis for the treatment of osteoarthritis,and explorethe mechanism of the sticking applying of Mongolian medicine.MethodsA total of 123 patients with osteoarthritis diagnosed in the Affiliated Hospital of Inner Mongolia Medical University from January 2017 to December 2017 were enrolled and divided into two control groups,Gutong strapping group and Glucosamine Hydrochloride Tablets group,41 patients of each group for 28 days of treatment.Touguling Rubber Ointment and Tianhe Gutong Strapping.Usage:clean the skin of the affected part,apply daily for 28 days of treatment.Glucosamine Hydrochloride Tablets.Take 0.75g once a day,2 times a day for 28 days.The therapeutic effects were evaluated before treatment,2 weeks after treatment and 4 weeks after treatment.All patients were followed up for 4 weeks.The scoring tables are VAS(Visual Analogue Scale/Score),the Mongolian medicinesyndrome andclinical symptoms and signs score,SF-36(the MOS item short SF-36 from health survey),JOA(Japanese Orthopaedic Association Scores),Assessment of shoulder joint dysfunction,Kofoed(Kofoed scoring standard),Harris(Hip Harris score),WOMAC(The Western Ontario and McMaster Universities Osteoarthritis Index),MEPS(Mayo Elbow Joint Function Scale),AOFAS(AOFAS Ankle-Hindfoot Scale).CGRP,TNF-a,MMP-3,VEGF and IL-10 were detected by ELISA method before and after treatment.Safety indicators were liver and kidney function,blood routine,urine routine and electrocardiogram.The auxiliary diagnostic index was X ray film.ResultsCompared with the baseline,the Mongolian medicine Touguling Rubber Ointment group improved the symptoms of pain,swelling,limited movement and daily life quality of patients in different degrees,in the VAS aspect,the Mongolian medicine Touguling Rubber Ointment group decreased significa ntly 2 weeks and 4 weeks after treatment(P<0.01),4 weeks of follow-up(P<0.05)compared with that before treatment.It shows that Touguling Rubber Ointment can significantly improve the pain symptoms of patients with osteoart hritis,with short-term and long-term effects.Compared with the control group after 2 weeks,4 weeks of treatment and 4 weeks of follow-up,the Mongolian medicine Touguling Rubber Ointment group also showed better relief effect on bone and joint pain than the control group;in the aspects of WOMAC pain,stiffness and joint function score,there was a significant decrease 2 weeks(P<0.01)and 4 weeks(P<0.05)after treatment.Although there was no significant difference between 4 weeks after treatment and 4 weeks after follow-up(P>0.05),the scores of pain,stiffness and function decreased significantly,suggesting that Touguling Rubber Ointment can relieve knee pain,stiffness and joint motor function in a short period of time,and has a long-term effect.WOMAC pain score was significantly lower in the Touguling Rubber Ointment group than in the control group(P<0.05),further confirming the relief effect of Touguling Rubber Ointment on pain.Similarly,WOMAC stiffness score and joint function score also showed that Touguling Rubber Ointment was similar to or superior to the control group;it showed that in terms of SF-36 somatic pain and physiological function,the scores of the Mongolian medicine Touguling Rubber Ointment group were significantly higher after 4 weeks of treatment than before(P<0.01),and there was a significant increase after 4 weeks of follow-up compared with 4 weeks of treatment(P<0.05).In terms of mental health,vitality,social function and emotional function,there were significant differences between the two groups after 4 weeks of treatment and before treatment(P<0.05).There was no significant difference between 4 weeks of treatment and 4 weeks of follow-up(P>0.05).After treatment,the Mongolian medicine syndrome and clinical symptoms and signs score of the overall clinical efficacy of patients were compared.Among the 39 patients of the Rubber Ointment group,it cured 5 patients,had effect on 27 patients,be effective to 6 patients and ineffective to one patient with the improving trend during the treatment period.Compared with each other,the overall effect of Touguling group was close to that of the two control groups,and even better than that of the control group in terms of pain,improvement of joint motor function and improvement of life quality.After treatment,the levels of MMP-3,TNF-α,VEGF and CGRP in the Touguling Rubber Ointment group were significantly lower than those before treatment(P<0.05).ConclusionThe Mongolian medicine Touguling Rubber Ointment has a good curative effect in the treatment of osteoarthritis patients.It has analgesic and detumescent effects on osteoarthritis,and can improve the life quality as a whole.It is even better than the traditional Chinese medicine and the western medicine inthe aspects of pain,swelling and improving bone and joint function index.The medicine can inhibit the expression of MMP-3,TNF-a,VEGF and CGRP in serum,and up-regulate the trend of IL-10,thus alleviating inflammatory reaction.
