ObjectiveA randomized controlled clinical trial was conducted to study the treatment of primary dysmenorrhea with cold coagulation and blood stasis by electro-acupuncture combined with hot compress massage ball of Thai herbal medicine in order to alleviate the pain state of patients with menstrual dysmenorrhea.They were randomly divided into treatment group(electro-acupuncture combined with Thai herbal hot compress massage ball),control group Ⅰ(simple acupuncture treatment)and control group Ⅱ(Thai herbal hot compress massage ball),30 cases in each group.To reduce the impact on their daily life and work,and improve the quality of life of patients.MethodsFrom December 2017 to December 2018 in Phatthalung’ s acupuncture hospital outpatient service of Thailand,90 patients who follow the inclusion criteria of primary dysmenorrhea were included in the standard of 1:1:1 proportion,randomly divided into treatment group(including Thai herbal hot compress massage ball),control group 1(pure acupuncture treatment)and control group 2(Thai herbal hot compress massage ball),each group of 30 cases.After acupuncturing Shenshu and Ciliao(Shenshu From the functional point of view,the kidney is dysmenorrhea.)points and gaining qi,electric acupuncture was then connected and used,and density wave was applied.The intensity of stimulation was based on the tolerability of the patients,and the electric acupuncturing was kept for 30 minutes,once a day,for 3 consecutive days with a menstrual cycle,and then performing follow-up after a course of treatment.After the remaining needles pulled out,the Thai massage ball which had previously got heated up to 35-40℃ temperature was directly applied to patients on lower abdominal and lumbar repeatedly many times,each time about 30 minutes.The scores of dysmenorrhea abdominal pain,dysmenorrhea symptom of traditional Chinese medicine,COX dysmenorrhea symptom and VAS pain score before and after treatment were observed.Results1.This study is a randomized controlled clinical research,after statistics analysis,the treatment group(including Thai herbal hot compress massage ball),control group Ⅰ(pure acupuncture therapy)and control groupⅡ(Thai herbal massage ball)in age,duration,and degree of dysmenorrhea,menstrual cramps,TCM syndrome score,COX dysmenorrhea symptoms and VAS pain score comparison there was of no significant difference(P>0.050)and comparable.2.Comparative analysis of TCM symptoms of dysmenorrhea before and after treatment:Primary dysmenorrhea belongs to functional diseases and has no organic pathological changes.At the same time,the incidence of this disease is very high.Western medicine has limited means to treat functional diseases.It can only relieve symptoms by taking analgesics or hormones for a short time.However,the side effects of taking drugs make some people hesitate and seek the help of doctors of traditional Chinese medicine.There was no significant difference in TCM symptoms of dysmenorrhea in the treatment group(electro-acupuncture combined with Thai herbal hot compress massage ball)except bed rest after the first course of treatment(P>0.050).The TCM syndromes of dysmenorrhea in the treatment group improved significantly after 3 courses of treatment and 3 months of follow-up(P<0.050).The score ofdysmenorrhea syndrome in control group Ⅰ(simple acupuncture therapy)improvedafter one course of treatment,except for abdominal pain,pale complexion and bed rest,there were significant or very significant differences(P<0.050,P<0.010).After three courses of treatment and three months of follow-up,the scores of dysmenorrhea syndrome were significantly different(P<0.050,P<0.010).In control group Ⅱ(Thai herbal hot compress massage ball),the score of dysmenorrhea syndrome before and after treatment was improved after one course of treatment.Except for paleness,cold limbs and bed rest,there were significant or extremely significant differences(P<0.050,P<0.010).After three courses of treatment,there were significant or extremely significant differences in dysmenorrhea syndrome(P<0.050,P<0.010).After three months of treatment,the score of dysmenorrhea syndrome was improved.There were significant or extremely significant differences in the pallor of TCM symptoms of dysmenorrhea except outside(P<0.050,P<0.010).There were significant differences in symptoms and time(P<0.001),and there were significant differences among different groups(P<0.001).Comparing the two groups,the difference of the three groups has statistical significance.It can be concluded that the effect of electro-acupuncture combined with Thai herbal hot compress massage ball group is the first,the simple acupuncture group is the second,and the Thai herbal hot compress massage ball group is the third.3.Cox pain and VAS scores before and after treatment compared with repetitive measure analysis of variance:Cox dysmenorrhea before and after treatment was statistically significant difference(P<0.001),there are differences in different groups(P<0.001).The conclusion comes from comparing the two groups that electroacupuncture combined with Thai herbal hot compress massage ball and simple acupuncture group were of statistically significance(P<0.050),which can be regarded that electroacupuncture combined with Thai herbal hot compress massage ball is superior to the simple acupuncture group.There were statistically significant differences in VAS at different time points(P<0.001)and among different groups(P<0.001).In pair-to-pair comparison,the differences of the three groups were statistically significant.The effect of electroacupuncture combined with Thai herbal hot compress massage ball group was the first,the acupuncture group was the second,and the Thai herbal hot compress massage ball group was the third.The differences were statistically significant at all times except after treatment and 6 months after treatment.4.The differences in symptoms and time were statistically significant(P<0.001),and the differences between different groups were statistically significant(P<0.001).In pair-to-pair comparison,three groups’ differences were statistically significant,electroacupuncture combined with Thai herbal hot compress massage balls can be thought of the first,simple acupuncture group the second,Thai herbal hot compress massage ball group the third;Each of the time differences were statistically significant,and the order of severity from high to low was ranked:before treatment,after 1 course of treatment,after 3 courses of treatment,followed up for 3 months.The time difference of dysmenorrhea degree was statistically significant(P<0.001),and the difference between different groups was statistically significant(P<0.001).There were statistically significant differences between the massage group and the massage+acupuncture group(P<0.001)and the acupuncture and moxibustion group(P=0.015).The differences were statistically significant at all times except 3 courses of treatment and 6 months after treatment.5.General data analysis:90 patients with primary dysmenorrhea,81 cases(90.0%)were actually completed,with a drop rate of 10%.Among them,2 cases(93.3%)dropped out by electro-acupuncture combined with hot compress massage ball of Thai herbal medicine(treatment group),3 cases(90.0%)dropped out by acupuncture alone(control group I),and 2 cases(93.3%)dropped out by self-conscious abdominal pain(treatment group),and 3 cases(90.0%)by self-conscious abdominal pain(control group I).When taking analgesics,they could not receive treatment on time because of working reasons,because they moved to other places and quit the study.Four cases(86.7%)of Thai herbal hot compress massage balls(control group II)fell off(because they were far away from the hospital and had repeated symptoms,because of conscious abdominal pain,they took analgesics on their own,because of working reasons,they could not receive treatment on time and their symptoms did not alleviate).There was no significant difference in age,course of disease and degree of dysmenorrhea between treatment group(electro-acupuncture combined with Thai herbal hot compress massage ball),control group Ⅰ(simple acupuncture therapy)and control group Ⅱ(Thai herbal massage ball)(P>0.050).6.Thai herbal hot compress massage ball has been proved to be effective in the treatment of muscle ache,pain of natural childbirth,injury of fall and so on.It can promote the balance recovery of body,mind and mind,and maintain the normal operation of organs and systems.Thai herbal hot compress massage ball improves the expansion of subcutaneous capillaries,promotes blood circulation,relieves pain through menstruation,and reduces muscle tension,which is consistent with the treatment of primary dysmenorrhea of cold coagulation and blood stasis type.However,at present,there is no systematic clinical study on the treatment of primary dysmenorrhea with Thai herbal hot compress massage ball,and the clinical effect,value and safety of the treatment of primary dysmenorrhea have not been discussed.Our study preliminarily confirmed the therapeutic effect of Thai herbal hot compress massage bal l on primary dysmenorrhea,and provided a new adjuvant treatment for primary dysmenorrhea.Although primary dysmenorrhea has been extensively studied in western medicine and traditional Chinese medicine clinic in recent years,there is still a lack of significant and widely used treatment methods,as well as a safe and highly acceptable treatment program.According to the characteristics and pathogenesis of primary dysmenorrhea of cold coagulation and blood stasis typo,we selected acupoints and used electro-acupuncture combined with Thai herbal hot compress massage ball to treat primary dysmenorrhea,which effectively improved the control effect of symptoms,and had a higher safety.In addition,it had a higher acceptance for the improvement of female spirit,skin and the comfort of supervisors.Our study provides an efficient,safe and highly acceptable treatment for primary dysmenorrhea of cold coagulation and blood stasis type,which may play an important role in promoting the treatment of primary dysmenorrhea by traditional Chinese medicine.In this study,the main indicators were subjective pain and TCM symptom score,both of which had certain subjectivity.There was no in-depth evaluation of hormone changes,prostaglandin PGE2 and other pain and primary dysmenorrhea symptoms related mediators before and after treatment,and no evaluation of Hematology and central nervous system related changes before and after treatment by imaging or B-mode ultrasound.Therefore,this study can not further explore the mechanism of electro-acupuncture combined with Thai herbal hot compress massage ball in the treatment of primary dysmenorrhea,which is relatively inadequate in depth.In this study,the therapeutic effect of electro-acupuncture combined with Thai herbal hot compress massage ball on primary dysmenorrhea can not be evaluated for a short time.In the follow-up study,this study needs to further explore the therapeutic value of electro-acupuncture combined with Thai herbal hot compress massage balls for primary dysmenorrhea through the following aspects:(1)carry out multi-center large sample clinical research,improve evaluation indicators,increase objective evaluation indicators,such as blood viscosity,prostaglandin PGE2 level and other indicators;(2)explore more long-term treatment options.(3)To explore the possible mechanism of electro-acupuncture combined with Thai herbal hot compress massage ball in the treatment of primary dysmenorrhea,to further explore its application value,to clarify indications and contraindications,and to provide more sufficient clinical and basic research basis for its clinical application and promotion.Generally speaking,although this study is only a preliminary exploration,the treatment of primary dysmenorrhea by electro-acupuncture combined with Thai herbal hot compress massage ball shows better application effect,and has higher safety and acceptability.Compared with the traditional single treatment method,it has more significant advantages,has better clinical popularization potential,and on the basis of full clinical syndrome differentiation and treatment.It can be widely promoted and provides an important alternative for the intervention of primary dysmenorrhea.ConclusionTreatment of primary dysmenorrhea of cold coagulation and blood stasis type by electro-acupuncture combined with hot compress massage ball of Thai herbal medicine.At present,the study of acupuncture combined with Thai herbal medicine for primary dysmenorrhea is relatively few.Our study confirms the therapeutic effect of electro-acupuncture combined with Thai herbal hot compress massage ball on primary dysmenorrhea.It is better than single electro-acupuncture combined with Thai herbal hot compress massage ball in the pain control of primary dysmenorrhea and the relief and regulation of Chinese medicine symptoms related to primary dysmenorrhea.It is suggested that the combined application scheme has synergistic effect,which can improve the therapeutic effect and the persistence of symptom control.Our study provides a new and applicable scheme for primary dysmenorrhea of cold coagulation and blood stasis type,which is the most common type of primary dysmenorrhea.Its therapeutic effect is accurate and safe.The toxic side effects and allergic reactions of external application of electro-acupuncture and hot compress massage ball with Thai herbal medicine are relatively less and have high application value.In fact,the priniciple of electro-acupuncture combined with Thai herbal hot compress massage ball for primary dysmenorrhea treatment is similar to that of acupuncture combined with important hot compress treatment in recent years,and can also be classified as acupuncture combined with important hot compress treatment.Thai herbal hot compress massage ball is generally used to relieve pain for women with dysmenorrhea.Thai massage is characterized by the use of Thai herbal hot compress massage ball.Among the similarities between Thai herbal medicine and Thai massage,the most characteristic medicinal compress massage is called Luk PraKob.Ingredients:(1)Bangladesh root:prevent wound infection,alleviate muscle soreness,protect skin,regulate blood,relieve itching,relieve arthritis and purple spot.(2)Kafirlam:sweating and detoxifying,alleviating foot odor,activating blood circulation and removing blood stasis,moisturizing skin,relieving muscle soreness,treating dizziness,insomnia,calming and tranquilizing the mind.(3)Tamarind:Activate blood circulation,treat colds,relieve cough,relieve diarrhea,relieve itching,moisturize skin,is commonly used in dermatology.(4)Curcuma Tonga:delaying aging,enhancing immunity,preventing wound infection,alleviating muscle soreness,maintaining skin,regulating blood,relieving itching and treating acne are commonly used in dermatology.Ginger:Prevent hair loss,relieve itching,treat urticaria and spider bites.Camphor Tree Crystal:Awakening the brain,can method.In the Thai Medical Department of the Thai Hospital,the treatment of women’s menstrual abdominal pain has increased the Thai herbal hot compress massage ball massage treatment.Hot compress can relieve stiffness,soreness,tense muscles and ligaments,back pain,arthritis,chronic pain or injury,viscera,skin diseases,headache,dysmenorrhea,long-term tension or anxiety and other discomforts.Herbal hot compress can also increase the healing ability of medicinal materials.Traditional Thai medicine can relax both body and mind,and help to relax muscles,so as to restore muscle function.Thai massage prescription has two formulas.This is a traditional formula widely used in Chiang Mai area and WAT PHO in Bangkok temples.But traditional Thai doctors not only use these two herbal formulas,but also prepare prescriptions according to their customers’ personal needs,such as arthritis,cold,fever and other diseases.The most masseurs have their own favorite and frequently used "Thai herbal medicine" hot compress ball.(1)With the passage of time after treatment,the scores of dysmenorrhea symptoms and VAS in the three groups decreased significantly,and reached the most significant clinical effect in the third course of treatment,i.e.the best short-term effect;and the scores of dysmenorrhea symptoms in the follow-up period,VAS score and treatment after the three courses of treatment,no significant abnormalities,indicating that the long-term effect of the three treatment methods is better.These results suggest that all three treatments can significantly improve the quality of life of dysmenorrhea patients during menstruation.Electro-acupuncture combined with Thai herbal hot compress massage ball has definite curative effect on treating primary dysmenorrhea,cold coagulation and fatigue syndrome.It can improve the degree of abdominal pain,TCM syndrome and the average severity of dysmenorrhea symptoms,shorten the duration of abdominal pain and the total time of onset of dysmenorrhea symptoms.(2)Among the initial,short-term and long-term effects of the three groups,the total effective rate of the treatment group was higher than that of the two control groups,while the short-term and long-term effects of the two control groups were comparable.The above shows that the therapeutic effect of electro-acupuncture combined with Thai herbal hot compress massage ball in the treatment group is better than that of the two control groups,suggesting that electro-acupuncture combined with Thai herbal hot compress massage ball in the treatment of primary dysmenorrhea of cold coagulation and blood stasis type is worthy of clinical promotion.Acupuncture and moxibustion,Thai herbal hot compress massage ball has significant clinical efficacy in treating primary dysmenorrhea,cold coagulation and blood stasis syndrome,and no adverse reactions have been found.It is simple,fast,safe and effective,and can be widely applied in clinic.
