Purpose:1.Theoretical research: the theory of kidney essence collection and zang image is the important proposition of the basic theory of traditional Chinese medicine.By sorting out the historical source of the development of the theory of kidney essence,the historical origin of the theory of kidney essence is deeply understood.Kidney essence collection is the main physiological function of kidney.Based on the theory of kidney essence collection,this paper discusses the system of "kidney-bone-marrow-brain" and the theoretical basis of "kidney essence deficiency syndrome".2.Experimental research: By the high-throughput sequencing technology of spectrum chips of the whole human genome expression,screening out the differently expressed genes among "disease"(senile dementia and primary osteoporosis)and "the same syndrome"(kidney essence deficiency syndrome),discussing the mechanism of molecular biology of kidney essence deficiency syndrome,verifying the scientific hypotheses of this study that "the mechanism of molecular biology of kidney essence deficiency syndrome may be closely related to the difference in the gene expression of the whole human genome of patients with senile disease(primary osteoporosis and senile dementia)",and discussing the influence of gender factor in patients with senile dementia and primary osteoporosis of kidney essence deficiency syndrome,and revealing the internal rules of the aging mechanism of senile diseases of "kidney-essence deficiency ",and enriching the basic scientific connotation of " kidney-essence collection".Materials and Methods:1.Theoretical research: By reading ancient Chinese medical books,sorting out the historical source and flow of the development of the theory of "kidney essence collection",deeply discussing the theoretical basis of "kidney essence collection,in charge of bone and marrow generation" and "kidney essence deficiency syndrome".2.Experimental study :(1)select 16 cases of senile dementia of kidney essence deficiency syndrome(AD group),16 cases of primary osteoporosis of kidney essence deficiency syndrome(OP group),and 12 cases of healthy elderly(control group)in the same age group that meet the inclusion criteria,and the three groups are equally divided between men and women.(2)based on the high-throughput sequencing technology of spectrum chip of the whole human genome expression,using the spectrum chip of gene expression of Prime View ? Human Gene Expression Array of Affymetrix to detect the gene expression patterns of the peripheral blood leukocyte among the three groups.(3)by contrasting AD group of kidney essence deficiency syndrome with control group,and OP group of kidney essence deficiency syndrome with control group to screen out the genes with different expressions.(4)according to gender differences,to compare the gene expression profile of AD group and the corresponding control group of kidney essence deficiency syndrome in male and female patients respectively,screening out the differently expressed genes,and furtherly discussing the characteristics of different gene expression in patients with different gender of the same disease and syndrome.(5)according to the gender difference,the gene expression profiles of the OP group of kidney essence deficiency syndrome of male and female with the corresponding gender control group respectively,screen out differently expressed genes,and furtherly compare the characteristics of the different gene expression in patients with the same disease and syndrome but different genders.(6)using bioinformatics methods to apply Gene Ontology(GO)functional enrichment analysis and Pathway analysis in genes of different expressions that has been sorted out.(7)q RT-PCR is used to verify the differently expressed genes.(8)furtherly analyze the homology of differently expressed genes in the "different diseases"(senile dementia and primary osteoporosis)and "the same syndrome"(kidney essence deficiency syndrome).Results:1.The historical origin of "the kidney essence collection" theory: pre-qin is the establishment period of the theory of "the kidney essence collection",qin-han and northern-southern dynasties is the inheritance and development period of the theory of "the kidney essence collection",tang-song-jin-yuan period is the further-develoment and enrichment period of the theory of "the kidney essence collection" ming-qing dynasties is the period of increasing maturity of the theory of "the kidney essence collection",modern period is the inheritance and innovation period of "the kidney essence collection".2."the kidney essence collection and in charge of born and marrow generation" theory is based on the anatomical form of kidney that in charge of body,take essence,menstruation,qi,body fluid as material foundation for life,and the physiological characteristics of kidney sealing as the functional foundation,and the kidney firmness as thinking-activity foundation,and kidney,urinary bladder,triple energizer and channels as human-body-as-a-whole-view foundation,and kidney and nature and social contact as integration of nature and human foundation which is the complex and unique theoretical system.3.The establishment of the system of "kidney-bone-marrow-brain" is based on the theory of "kidney essence collection and in charge of bone and marrow generation".The pathogenesis of both senile dementia and primary osteoporosis are based on "kidney essence deficiency",and belong to the same syndrome of kidney essence deficiency-"different diseases and the same syndrome".4.Differently expressed genes in patients with senile dementia of kidney essence deficiency syndrome:(1)compared with the control group,there are 308 probes specifically expressed in AD patients of kidney essence deficiency syndrome,corresponding to 247 genes(FC?1.5,q-value?5%),of which 228 are up-regulated and 19 are down-regulated.There are 6 genes whose up-regulation are more than 3 times,namely S100 P,SAMSN1,CCL4,TNFAIP3,SDCBP and IL1 B,and 3 genes whose down-regulation is less than 0.5 times,namely CX3CR1,EGR1 and FOS.After q RT-PCR verification,compare the 6 up-regulated genes with control group,the level of expression has statistical significantce(p < 0.05).Compare the 3 down-regulated genes with control group,the level of expression has statistical significantce(p < 0.01).(2)among the selecting differently expressed genes by comparing AD patients of kidney essence deficiency of both male and female with corresponding control group,both show up-regulated gene of different expression(FC?1.5,q-value?5%)is: S100 P.5.Differently expressed genes in patients with primary osteoporosis of kidney essence deficiency syndrome :(1)compared with the control group,910 probes are specifically expressed in OP patients of kidney essence deficiency syndrome,corresponding to 602 genes (FC?1.5,q-value?5%),of which 579 are up-regulated ones and 23 are down-regulated ones.There are 7 genes which are up-regulated for more than 3 times,namely S100 P,SNX3,DEFA1,MMP9,LTF,ANXA3 and IL1R2.Three are three genes which are down-regulated for less than 0.5 times,including CX3CR1,SPON2 and GNLY.After q RT-PCR verification,the expression of 7 up-regulated genes is statistically significant compared with those of the control group(p < 0.05).The expression of 3 down-regulated genes is statistically significant compared with that of the control group(p < 0.05).(2)among the differently expressed genes which are screened out between OP patients of kidney essence deficiency syndrome and corresponding control group of male and female,both of them presenting different genes that are up-regulated.(FC?1.5,q-value?5%)are SNX3.6.The homological analysis of differently expressed genes in patients of "different disease"(senile dementia and primary osteoporosis)and "the same syndrome"(kidney essence deficiency syndrome): compared with the control group,both OP and AD patients of kidney essence deficiency syndrome presenting different genes that are up-regulated is S100 P,both presenting different genes that are down-regulated is CX3CR1 compare the three groups of differently expressed genes screened out in the OP of male,female and the whole OP of kidney essence deficiency syndrome with the corresponding AD of kidney essence deficiency syndrome,three groups all presenting different genes that are up-regulated is only TNFSF10;all presenting different genes that are down-regulated is only CCL4/CCL4L1/CCL4L2.Conclusion:1.The zang image theory of "kidney essence collection" is formed in the practice process of the Chinese nation's long-term maintenance of life and health,and struggle with diseases.Through continuous inheritance,development,perfection and innovation,it has become a unique theoretical system.The theory of "kidney essence collection,in charge of bones and marrow generation" is an important part of the physiological function of "kidney essence collection".2.The establishment of the system of "kidney-bone-marrow-brain" is based on the theory of "kidney essence collection,in charge of bones and marrow generation".Both senile dementia and primary osteoporosis take "kidney essence deficiency" as the fundamental pathogenesis and belong to kidney essence deficiency syndrome of "different diseases and the same syndrome".3.The different genes that are significantly expressed in patients with senile dementia of kidney essence deficiency syndrome are S100 P,SAMSN1,CCL4,TNFAIP3,SDCBP,IL1 B,CX3CR1,EGR1 and FOS,in order.The different genes that are significantly expressed in patients with primary osteoporosis of kidney essence deficiency syndrome are S100 P,SNX3,DEFA1,MMP9,LTF,ANXA3,IL1R2,CX3CR1,SPON2,and GNLY.For patients with the same disease and syndrome,the influence of gender factor on gene expression is obvious,indicating the objective existence of the influence of physical factors on disease.4.The common differently expressed genes of patients with senile dementia and primary osteoporosis of kidney essence deficiency syndrome are S100P(up-regulation),CX3CR1(down-regulatiom),suggesting the different gene expression of patients with senile diseases of kidney essence deficiency syndrome(senile dementia and primary osteoporosis)exists individuality as well as university,indicating the foundation of molecular biology of the kidney essence deficiency syndrome is related to the " university ".This " university " is mainly reflected in the abnormal expression of genes related to stem cells and their microenvironment.5.This study suggests that in clinical practice," the same syndrome of different diseases" requires not only the same syndrome differentiation,but also the different disease differentiation,as well as taking the influence of physical and other factors on diseases into account in order to furtherly provide macro and micro basis for the accurate " treatment of diseases and syndrome combination" in the future.
