Nowadays, the dimension of the industries are getting larger and larger, and the harmful substances and energy of production systems are increasing, so there are more and more risks in this area, the causes of the accidents are more complicated, the c-onsequence of the accidents are getting serious. For identifying the hazards of the syst em, protecting peoples'health and safety, decreasing the damage of the accidents, vari ous safet-y assessment methods are constantly improved and perfected, to provide scie ntific basis of safety management decisionmakeing.In this article, a prototype of an integrated hazard analysis system-HAZOP/FTA has been developed. Two widely accepted hazard assessment procedures, HAZOP, FTA, can be performed in evaluating the safety of the plant. The major work and conclusions are as follows.(1)The characteristics of common safety assessments had been analysed, after the comparing we ensure that the using HAZOP method to indentify the hazard of the plant and FTA method to support the evaluation. Using this model we can improve the traditional mathod in partition of the unit, logical computation, qualitative analysis and quantitative analysis.(2)The logical process of the HAZOP/FTA model, the measure of improving the traditional HAZOP method, and the mathematics modelhas been carried out. The fuzzy problem of the data information was solved by using triangular fuzzy sets theory, and HAZOP/FTA was set up using Visual Basic 6.0.(3)A knowledgr base for HAZOP/FTA was set up using Visual Basic+SQL Server including reasons of deviation database, results of deviation database, safety compensate measure of deviation database, failure rate of equipments data base, data management module, FTA computation module and HAZOP analysis outcomes module.(4) The user interface of HAZOP/FTA was achieved, which was convenient for the user to input and select data and compute.(5)The HAZOP/FTA was applied in the case-the DAP process system. The results had been compared with traditional HAZOP. We can see that the system analysis is more comprehensive, the data results are more accurate and reliable, and this safety assessment has good applicability. (6)The HAZOP/FTA model can be used in prto-chemical production, storage installation and etc. It can provide qualitative estimate, quantitative estimate, and give advice to the operators...
