Effect Of Jiangtang Decoction On Vascular Endothelial Cell Function In Type 2 Diabetes Mellitus Patients

Objective:Through intervention of Chinese or western drugs in patients with type 2 diabetes mellitus, to evaluate the efficacy of Jiangtang Xiaozhi particles in treatment of type 2 diabetes mellitus, and to explore the efficacy and mechanism of this drug in protecting the function ofendothelial cell of type 2 diabetes patients.Method:Adopting randomized, parallel-control methods trail design.35 T2DM patients of qi-yin deficiency and phlegm and blood stasis syndrome, were randomly assigned to two groups, treatment group (18 cases), the control group (17 cases).Treatment group was given the conventional treatment combined with Jiangtang Xiaozhi particles, the control group was given the conventional treatment plus metformin sustained release tablets, and the test cycle is 12 weeks. Before and after the test the body weight, body mass index (BMI), waist circumference, TCM symptom score were observed, fasting blood glucose(FBG) and 2 hours postprandial blood glucose(PBG), glycosylated hemoglobin(HbA1c). serum total cholesterol(TC), triglycerides(TG), high-density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol (LDL-C), hypersensitive creactive protein(hs-CRP), nitricoxide(NO), endothelin(ET),fasting plasma insulinand (FINS), and the HOMA-IR were measured. SPSS 17.0 was used to analysis those data.Result:The amount of cases of this study in final is 33, the treatment group (17 cases), the control group (16 cases).①The level of patients’ body weight, waist circumference, BMI index of treatment group and control group after 12 weeks intervention are lowered than the level before intervention, and the difference is statistically significant (P< 0.05).②The effective rate in improving TCM syndromes of treatment group was 70.6%,which is better than that of control group 56.3% (P< 0.05), through the comparison TCM symptom scores, we found that the efficacy of Jiangtang Xiaozhi particles in improving short-breath and fatigue, is better than that of control group(P< 0.05). ③The level of HbA1c、FBG、PBG、 FINS、HOMA-IR of the two groupswithin group comparison or incomparison between groups is not statistically significant t (P>0.05).As well as, we assorted the patients into another two groups based on the length of course of disease, we found that the level of HbA1c、FBG、 FINS of patients whose course of disease less than 12 months in the treatment group after intervention, is less than before intervention(P<0.05). ④The level of TC,TG,LDL-C, hs-CRP of the two groupswithin group comparison is not statistically significant (P>0.05).⑤The level of NO of the two groupswithin group comparison is not statistically significant(P>0.05). The level of ET of the two groupswithin group comparison is statistically significant within group comparison (P<0.05), which is less than the level before intervention, but the level of ET incomparison between groups is not statistically significant (P>0.05).No severe adverse event occurred in the process of the study. The amount of cases of this study in final is 33, the treatment group (17 cases), the control group (16 cases).Conclusion:Jiangtang Xiaozhi particles Can obviously relieve the clinical symptoms and signs of traditional Chinese medicine in T2DM patients,and can obviously decrease the plasma ET levels, has the function of protecting endothelial function in patients with type 2 diabetes.
