Clinical Observation Of Ulcerative Ulcer Damp - Heat Stasis Syndrome

Objective:To explore the possible action mechanisms of Ulcer Oil in promoting diabetic foot ulcer healing. To further clarify the effect of Ulcer Oil on promoting diabetic foot ulcer healing. To provide more clinical evidences for external therapies of Traditional Chinese Medicine external treatment therapy for diabetic foot.According to the good effect of Ulcer Oil in the clinical treatment of diabetic foot ulcer, observed a randomized controlled trial of ulcer oil and ethacridine lactate for external application in the treatment of diabetic foot ulcer with syndrome of damp heat and blood stagnation.Methods:50 cases of patients of diabetic foot ulcer were randomly divided into treatment group (traditional Chinese medicine group) and control group (Western medicine group), each with 30 cases. On the basis of routine therapy of internal medicine treatment and wound debridement, the treatment group applies external treatment with Ulcer Oil, the control group using applies external treatment with ethacridine. The changes of ulcer area, necrotic tissue, granulation tissue, epithelial tissue, inflammatory infiltration, exudation, the pain of two groups of patients before and after the treatment were observed, and the changes of ABI before and after treatment, transcutaneous oxygen pressure and blood WBC, NE%, CRP were also included. Digital camera records of ulcers were took before and after treatment, and the wound area, the area of necrotic tissue and granulation tissue, inflammatory infiltration area were calculated by Image J software.Results:The total effective rate of treatment group was 86.96%, and the control group was 73.91%. The treatment effect of treatment group was significantly better than the control group in the treatment of diabetic foot ulcer (p<0.05). The average reduction rates of wound area of the treatment group and the control group after treatment were respectively 49.80% and 37.15%. The average reduction rates of necrotic tissue after treatment of the treatment group and the control group were respectively 67.11%and 50.85%. The average granulation tissue growth rates of the two groups after treatment were 62.31%in the treatment group and 41.85%in the control group. The average inflammatory infiltration reduction rates of two groups after treatment were 82.10%in the treatment group and 59.39%in the control group. The average epithelial tissue coverage rate of the two groups after treatment were 61.72%in the treatment group and 42.56%in the control group. The treatment group was superior to control group in those aspects of narrowing ulcer area, reducing the necrotic tissue, promoting growth of granulation tissue and epithelium (p<0.05). The wound exudation and pain of two groups after treatment were reduced compared with those before treatment (P<0.05), and the two groups have no significant difference (P>0.05). The treatment group and the control group both showed no significant improvement of the ABI value (p>0.05).The transcutaneous oxygen pressure values of the treatment group and the control group both got a certain improvement. The peripheral micro circulation were both improved in the two groups after treatment, and the treatment group was significantly better than the control group (p<0.05). WBC and CRP in the treatment group after treatment were lower than before treatment (p<0.05), NE% was not significantly reduced, WBC, NE% and CRP had no obvious change before and after treatment in the control group.Conclusion:(1) The patients of diabetic foot ulcer mostly have the characteristics as follows: much older age, long duration of diabetes,delayed treatment of diabetic foot ulcer, more complications and neopathies,etc (2)Ulcer Oil has good promotion for diabetic foot ulcer healing, mainly to promote reducing wound necrotic tissue and exudate, promote growth of granulation tissue and epithelial tissue, reduce inflammatory infiltration, ameliorate wound pain, improve transcutaneous oxygen pressure value, improve topical microcirculation of wound and around wound, significantly reduce the wound area. Ulcer Oil has effects of anti-infection, anti-inflammatory, acesodyne, improving microcirculation, promoting the absorption of necrotic tissue, promoting the growth of granulation and epithelium.
