Knee osteoarthritis(knee osteoarthritis KOA)is a chronic progressive arthriti-s,which pathological characteristics is articular cartilage degeneration of knee joi-nt and bone hyperplasia formation around the joint,mainly for the knee joint pain, stiffness,movement disorder and so on.KOA may be suffered from youth to old ag-e,above 50 years old is most common.The pathogenesis of KOA are not yet compl-etely elucidated,there are still many hypothesis.Postmenopausal knee osteoarthrit-is occurs is very common, which is a special, multiple types of KOA.The study in-cludes two parts,one is the modern medicine and traditional Chinese medicine res-earch progress in the treatment of KOA.and the other one is the clinical study on the treatment of postmenopausal knee osteoarthritis by using of the prescription of Bu Shen Huo Xue.Objective To study serum interleukin 1(IL-1),tumor necrosis factor α (TNF-α),Estradiol(E2),Dickkopf-1(Dkk-1) level and cure effect in postmenopausal knee osteoarthritis with self-made Bu Shen Huo Xue which is nourishing kidney and removing blood stasis.Methods 60 patients with knee osteoarthritis which meeting the diagnosis criteria were randomly divided into treatment group and control group,each of the 30 people.The treatment group were given the traditional Chinese medicine-Bu Shen Huo Xue a dose and twice daily;control group were taken Diacerein Capsule for 50mg/d,the observation period was 4 weeks.Formulate case report form and re-cord the relateing information of two groups of patients before and after treatment. And fasting venous blood was taken in the morning of all patients before and after treatment to detect the content of IL-1,TNF-α,E2,Dkk-1.Results In treatment group,the controlled and remarkable effective rate is 60-% and the total effective rate is 96.67%.While in the control group,the controlled and remarkable effective rate is 53.33% and the total effective rate is 93.33%.Bef-ore and after treatment,the total scores comparison shows,the two groups compare with their before treatment were very significant difference (P<0.01),the two grou-ps after treatment there was no significant difference between groups(P>0.05).The two groups before and after treatment of serum IL-1 and TNF-α within group co-mparisons showed that there was very significant difference(P<0.01);Comparison of the two groups after treatment between the group showed that IL-1 had no sign-ificant difference(P>0.05),TNF-a content had significant difference(P<0.05).Abo-ut serum E2,the treatment group before and after treatment had very significant di-fference(P<0.01),the control group before and after treatment had no significant difference(P>0.05),comparison after treatment between the group showed there w-as very significant difference(P<0.01).Serum Dkk-1,there were no significant diff-erences within the two groups before and after treatment,and the same result bet-ween groups after treatment.The correlation analysis of the serum E2 with IL-1,T-NF-a,DKK-1 in two groups after treatment showed that there was no significant correlation between treatment group and control group after treatment(P> 0.05).Conclusion 1.The main pathogenesis of postmenopausal KOA is weakness of kidney and blood stasis,the traditional Chinese medicine Bu Shen Huo Xue can well improve the symptoms of postmenopausal knee osteoarthritis.2.The prescript-ion of Bu Shen Huo Xue can intervene postmenopausal KOA by regulating the lev-Is of estrogen and inflammatory cytokines.3.The prescription of Bu Shen Huo Xue had no obvious effect for Dkk-1 of Wnt signal pathway,Wnt signal pathway may n-ot participate in Bu Shen Huo Xue decoction intervention way.
