Investigation On TCM Syndromes Of Primary Bile Reflux Gastritis And Its Correlation With Gastric Mirror Image

Objective:Bile reflux gastritis (BRG) is a kind of difficult to treat gastrointestinal diseases. With the extensive application of clinical digestive endoscopy, the detection rate was significantly higher. Domestic and international medical community is increasingly concerned about and pay attention to it. At present, a lot of modern medical treatment of BRG adopt comprehensive therapy,and there is no specific medicine.Although it can temporarily alleviate clinical symptoms,but the disease is easy to relapse and has a poor long-term curative effect. TCM has a unique advantage in the fight against BRG and the consolidation effect. However, clinical still lack of unified understanding and diagnostic criteria. Although TCM has achieved some therapeutic effect,but it is difficult to promote in clinical practice.This study is designed to investigate the distribution regularity of TCM syndrome types of BRG and the correlation with endoscopic pathological manifestations. Help clinicians to build the system of theoretical system and improve the specification of the dialectical method. In order to provide objective basis for TCM treatment of BRG.Method:Design a detailed clinical questionnaire. Record the general condition of patient, four diagnostic information and results of gastroscopy data. After the input of data,using SPSS19.0 statistical software analysis.Result:①The proportion of male and female patients is 1:1.67. The highest disease incidence rates is 51-60 years old. The course of 1-3 years is the largest proportion. The main predisposing factor is improper diet, and eat spicy and sweets are the most common. People previously suffered from biliary disease is closely related to the disease.②In the macroscopic syndrome differentiation, spleen-stomach weakness syndrome>Liver-stomach qi stagnation syndrome>spleen-stomach damp heat syndrome>liver-stomach heat syndrome>cold-heatmixed syndrome>stomach yin deficiency syndrome>stomach blood stasis syndrome. Through factor analysis of statistical method, the disease located mainly in the liver, spleen, stomach;The disease syndrome elements are mainly Qi stagnation,Qi inverse,Qi deficiency, Yang deficiency, heat, heat and humid. Syndrome classifications are spleen-stomach weakness syndrome,Liver-stomach qi stagnation syndrome, spleen-stomach damp heat syndrome, liver-stomach heat syndrome.③The patients of spleen-stomach weakness syndrome have a heavier bile reflux degree, with the HP infection rate is lower. The patients of non-spleen-stomach weakness syndrome has a lighter bile reflux degree,with the HP infection rate is higher.④The endoscopic pathological manifestation of Liver-stomach qi stagnation syndrome is mainly mucosal hyperemia.The spleen-stomach weakness syndrome’s is mainly cardia relaxation. The liver-stomach heat syndrome’s is mainly mucosal hyperemia. The spleen-stomach damp heat syndrome’s are mainly mucosal erosion and hyperemia. The stomach yin deficiency syndrome’s are mainly mucosal hyperemia and ulcer.The stomach blood stasis syndrome’s is mainly gastric mucosa old hemorrhage.The cold-heatmixed syndrome’s is mainly helicobacter swelling.When the gastric mucosa with congestion based, red tongue runs the highest percentage, followed by light dark tongue and dark red tongue. When the gastric mucosa with edema based, pale tongue runs the highest percentage, followed by pink tongue. When the color of mucus lake is light yellow and yellow, The tongue coating give priority to with thin white and white greasy. When the color is dark yellow and Kelly, The tongue coating give priority to with thin white and thin yellow.In the microscopic syndrome differentiation, liver-stomach heat syndrome>Liver-stomach qi stagnation syndrome>spleen-stomach damp heat syndrome>spleen-stomach weakness syndrome>stomach yin deficiency syndrome> stomach blood stasis syndrome. The highest conformity rate of macro and micro syndrome differentiation is the liver-stomach heat syndrome, followed by spleen-stomach damp heat syndrome and stomach blood stasis syndrome.Conclusion:①The weakness of spleen and stomach is the main cause of BRG. Treatment should be given priority to replenish qi and fortify the spleen.②There is a certain regularity between macro and micro syndrome differentiation. The theory of TCM clinical diagnosis has virtually penetrated into microcosmic syndrome differentiation, both of them are relevant and should be balanced.
