Objective:To assess the effect and safety of TXA on perioperative blood loss and the change of hemoglobin in bilateral total knee arthroplasty(TKA).Methods:We selected 69 patients who need bilateral TKA and divided them into 2 different groups randomly. The patients in the experimental group were given I.V. drip of 1g of tranexamie acid with 100 ml of 50g/L glucose solution, while those in control group were only given the same glucose solution. Total blood loss, intraoperative blood loss, the hidden blood loss, amount of post-operation drainage, the ratio of blood transfusion, D-dimer, PT and APTT were studied and compared. We also observed the dynamic changes of hemoglobin of the two groups.Results:The blood loss of the experimental group were significantly higher compared to the control group(t=2.194~2.908,P<0.05). The blood transfusion rate of experimental group was obviously lower(2=5.219,P=0.022). The hemoglobin of the experimental group were significantly higher than that of the control group on the third day as well as on the fifth day(t=-3.481~2.319,P<0.05).Conclusion:Tranexamic acid can significantly reduce the blood volume changes of the TKA patients, reduce the dynamic change of hemoglobin, so it can reduce the need for blood transfusion while doesn’t increase the risk of developing DVT.
