CLINICAL STUDY ON THE RELATIONSHIP OF HEAT-DAMPNESS SYNDROME TO PATHOLOGY OF RENAL BIOPSY OF IgA NEPHROPATHY

Abstract Background: A large number of data of clinical study indicate that manifestation of heat-dampness syndrome often exists on a certain phase or during the whole course of IgA nephropathy. Objectives: With the study on the relationship of heat-dampness syndrome to pathology of renal biopsy of IgA nephropathy and immune deposition and clinical syndrome to explore the mechanism of heat-dampness syndrome of IgA nephropathy. Materials and Methods: 61 patients of primary IgAN were selected as research subjects, among which 31 are male and 27 are female, with an average age of 34. 19±11.7 years old. 29 belong to obvious heat-dampness, group of which 15 are male, 14 are female, with an average age of 36. 17± 12.9.32 belong to unobvious heat-dampness, group of which 19 are male, 13 are female, with an average age of 36. 16 ±13.O.the renal biopsies were examined by light microscopy, electron microscopy and immune of lureeenee microscopy. Results: (1) there is no linear correlation between heat-dampness integral and immune deposition, glomerular lesions, the degree of glomerular mesangial cells proliferation, Ser, BUN concentration. But heat-dampness integral and the tubulointerstitium lesions are in linear correlation. (2) Glomerular mesangial cells proliferation hasn't obvious heat-dampness manifestation during mild to moderate degree. But after mild to moderate degree period, increase trend is obvious. (3) Tubulointerstitium lesions of obvious heat-dampness syndrome were significantly higher than that of the unobvious heat-dampness syndrome. (4) Heat-dampness integral of macroscopic hematuria in JgAN were significantly higher than that of the none macroscopic hematuria group. Glornerular lesions of macroscopic hematuria were significant lower than the none macroscopic hematuria group. And two group's immune deposition were no significant difference. Conclusions; (1) In the process of IgAN, manifestation of heat-dampness syndrome may be an indicator of activiting 2 of cell factor and inflammatory medium which is related to the damage of tubulointerstitium and the prognosis. (2) Macroscopic hematuria in JgAN may be a result of activating of heat-dampness syndrome. and play an important role in the prognosis. (3) The regularity of heat-dampness syndrome of IgAN lesions: As tubulointerstitium lesions were serious, heat-dampness integral were high; Macroscopic hematuria group always appear heat-dampness; Glomerular mesangial cells proliferation hasn't obvious heat-dampness manifestation before mild to moderate degree. But during mild to moderate degree period, heat-dampness is obvious.
