Objective: The accuracy of urodynamics for diagnosis of BOO has been discussed by comparing the differences between the method of suprapubic puncture and that of transurethral catheterization for pressurelow measurements during voiding of BPH patients. Patients and methods: Both the suprapubic puncture and the transurethral catheterization methods were adopted to measure pressurelows during voiding of twentyhree BPH men with a median age of 69. 3 years (range, 57 to 77) and the corresponding parameters were compared with test. Results: (l)Suprapubic puncture versus the transurethral catheterization: Qmax increased 1.19m1/s,P40-. Pdet,Qmax>40 and URAz29.OcmH2O were used as the criteria of obstruction. Cases numbers in the suprapubic group were respectively 18,16,22,23,18, while those were 19,20, 23,23,23 in the transurethral catheterization group. The former ones were obviously less than the latter. Conclusions: In pressurelow mearuments during voiding of BPH patients, there were significant differences betweeen the suprapubic and the transurethral methods. This illustrates that the resistance from the catheter influences the results. So, to BPH patients, we recommend the former in order to overcome this shortcoming. In other cases, it depend on different conditions to chose which of the two methods. Whichever method is to be choosen, we should make the patients understand and get their consents.
