Association Of Insulin Resistance Of Obesity And Its Relative Factors

Objective To study the correlation between insulin resistance index(Homa-IR) and body weight index (BMI), ratio of waistline to hipline ( W/H) , plasma lipid, uric acid (UA) , leptin, plasma renin activity ( PRA ), angiotensin 0 (AU)and aldosteroneC ALD) so as to determine the insulin resistance of obesity and its relative factors.Methods Different BMI, W/H, Homa-IR, triglyceride (TG), total cholesterol (TO,high density lipoprotein(HDL) ,low density lipoprotein(LDL) , U A, leptin, PR A, AH and ALD were measured. 110 cases were divided into two groups, the normal body weight group and the extra normal body weight or obesity group. Compare the differences of these items between the two groups. Analyse the correlation between BMI and Homa-IR,evaluate the correlation between BMI, W/H, Homa-IR and TG,TC, HDL, LDL,UA,leptin,PRA,AH ,ALD respectively. Using multiple linear regression method to determine the relative factors of Homa-IR.Results The difference between the normal body weight group and the extra normal body weight or obesity group is significant in TG, TC, HDL, LDL,UA, Homa-IR, leptin(P<0. 05). The correlation between Homa-IR and BMI, W/H, TG, LDL, UA, leptin are positive and so are the correlation between BMI and W/H, UA, Homa-IR, leptin and so are W/H and Homa-IR,BMI,LDL,UA,leptin(P<0. 05). THE correlation between HDL and BMI,W/H,Homa-IR are negative(P<0. 05) . THE correlation between leptin and Homa-IR is the strongest than others in men(r=0. 444,P<0. 001) and the correlation between leptin and W/H is the strongest than others in women(r= 0. 457,P<0. 01). The W/H, insulin, leptin is respectively relative to Homa-IR. There is no correlation between BMI,W/H,Homa-IR and PRA,AH ,ALD.Conclusion The extra body weight or obesity group are susceptible to insulin resistance, lipid metabolism disorder, high level of uric acid and leptin. The Homa-IR is related to the radio of waistline to hipline, insulin and leptin respectively. By reducing body weight and radio of waistline to hipline and eventually acquiring a lower level of leptin can improve the situation of insulin resistance and prevent the complication of o-besity.
