Objective: Prostatitis especially chronic abacterial prostatitis or chronic pelvic pain syndrome (CPPS ) is the most common disease of male genital system. It not only results in unwell of body, but also results in strong mental influence because of it's persisting. At present, we know little about CPPS's etiological factor, pathogenesis and pathophysiology. So it's therapy can only be empirical, and the effect is unsatisfied. Recently, there was an animal experiment demonstrate that: the transcript level of interleukin-4 (IL-4) elevated after given E2 for 4 weeks, but there is no report of elevating of human IL-4 level in express prostatic secretion(EPS) from man with chronic abacterial prostatitis. IL-4,or B-cell growth factor is the characteristic cytokine issued by Th2 cell. It's main effection is restraining cellular immunity and facilitating humoral immunity; enchancing specific and unspecific kill effection. Base it, We compared IL-4 level in EPS and NIH-CPSI score from man with chronic abacterial prostatitis with that from healthy man, analyzed the relationship between IL-4 level and symptoms of chronic pelvic pain syndrome, to discusse the function ofIL-4 in the process of prostatic inflammation, and try to find a shortcut to diagnose CPPS.Material and methods: 24 men with CPPS seen in our urology clinic and 16 normal men joined our study .The patients varied from 19 to 32 years(mean:24), and were classified by premassage and postmassage test offered by Nickel J.C. Controls varied form 24 to 27 years (mean: 26) . EPS samples were collected by digital rectal examination. After routine test of EPS ,the remainder of EPS was stored at -20 until thawed for analysis. At the same time, NIH-CPSI was used to evaluate the state of CPPS. Enzyme-linked immunoadsordent assays (ELISA) were used to measured IL-4.A11 samples were assayed for IL-4. All data were statistically analyzed by software SPSS.Result: IL-4: patients: 53.27+60.20(mean ?Std deviation);control: 35.62+22.45 (mean盨td deviation) . NIH-CPSI: 22.13+4.48 (mean+Std deviation) .There was no significant difference of IL-4 in EPS between CPPS and normal, and no correlation between IL-4 level and symptoms. (P>0.05)Conclusion: There is no significant difference of IL-4 between CPPS and controls, and no correlation between il-4 and symptoms. It shows that: the local immunologic reaction is not humoral immunity. Maybe it is cellar immunity. IL-4 has no leader function in the process of CPPS, and it's function maybe is a little party of whole cytokine network.
