PIURPOSE The research of TCM HUANG QI GUI ZHI FIVE MATTER CAPSULE on knee osteoarthritis, its effectivness in reliefmg pain clinically and its therapeutic improvement. Also the experiment on the effectiveness in cartilage protection in experimental animal. METHOD1. ClinicalAccording to the indices of severity and disease activity forosteoarthritis ( ISOA ), A group has samples with the rating 1-7 ISOA; Bgroup has cases with the rating 5=8 ISOA. Both TCM group and FENBID grouphave samples of A and B groups.TCM group: 36 cases, given the HUANG QI GUI ZHI FIVE MATTER CAPSULE orally,3 times a day, 3-4 tablets each time, one month for each treatment course,continues for 2 months, followed up after the third month.FENBID group: 27 cases, 2 times a day, each time with 0. 3g, continues from2 weeks to 2 months, followed up after the third month.2. ExperimentMaterials: Healthy SD rats, female, 3 months old, weight 200~300g. Method: They were severed of their right leg medial ligament; the cruciate ligament and the meniscus, which creating the unsteadiness knee joint. They were also resulting in ovariectomized . They were randomly divided into the TCM group, the FENBID group and the control group. From each respective groups were randomly sacrificed at 2, 4, or 8 weeks after therapy and subjected to various investigation.RESULTS1. Clinical researchTCM group A cure rate 57.9%, group B cure rate 5.9%.FENBID group A" cure rate 15. 4%, Group B" cure rate 7.1%.2. Experiment, research 2.1 Histological examinationTCM group : From the second week, the joint cartilaginous surface starts to peel off. Crevices, sloughing of surface cells and chondrocyte columns are seen. Few lacunae are present. Hyperplasia of the cartilaginous surface cells are seen from 4 weeks. Crevices are filled. The chondroid surface is still irregular. Chondrocyte alignment irregular and lacunae are increased. The 8th week shows sloughing of the chondroid surface, loss of surface cells and appearance of crevices. Many lacunae are seen at the growth plate, with irregular chondrocyte alignment, some in clusters. FENBID group : Obvious peeling off of cartilage is seen in the second week with many crevices. The cartilage layer is of variable thickness with irregular chondrocyte alignment. Seen from the 4th week are hyperplasia of the cartilaginous surface cells, repair of crevices, undulating chondroid surface, irregular alignment of chondrocytes and few lacunae. Seen from the 8th week are sloughing of cartilaginous surface layer, erosion, many crevices, irregular chondrocyte columns and few lacunae. CONTROL group : From the 2nd week are seen sloughing of surface cartilaginous layer, increased crevices, interrupted chondrocytes, cell death and few lacunae. From the 4th week are seen crevices widening and deepening, thinning of the chondroid layer, eccentric chondrocyte nuclei, necrosis of some chondrocytes and scarcity of lacunae. From the 8th week are seen obvious sloughing of cartilage up to the calcified layer and massive chondrocyte degeneration and necrosis. 2. 2 Plasma estradiol investigation The TCM group is significantly higher than the control and FENBID groups.CONCLUSIONThe TCM show therapeutic effect in improving mild and moderateosteoarthritis of the knee joint. Response to severe osteoarthritis seemsto be unsatisfactory.The TCM capsule can improve renal function. It also shows cartilage5protective effect in experimental animal models of osteoarthritis.
