| Objective: Mesangial proliferative glomerulonephritis (MsPGN) is the most commonly type of chronic nephritis,which leads to chronic renal failure.The main pathological characteristics of this disease is mesangial cellular proliferation and extracellular matrix(ECM) expansion.In this study,to explore the clinical effect of shenluotong(SLT) on curing MsPGN and its possible mechanism on effect which is lipid metabolism and blood coagulation mechanism.Method: In this study, 56 cases of MsPGN diagnosed by renal biopsy were treated with shenluotong therapy(therapeutic group) comparing with 30 cases of common threewingnut root poly-GLYcoside therapy (control group). To make the rat model of MsPGN by injecting staphylococcus enterotoxin B(0.4mg/kg) into rat caudal vein at the 8th and 15th day,feeding bovine serum albumin (20mg/d) in another day, injecting Freunds adjuvant(2ml/kg) periodical (first and 8th day). Forty rats were averagely devided in random into 4 groups including normal contrast, model, shenluotong and shenyanshu . Wetreated them with shenluotong, and at the same time used shenyanshu as control. To observe the changes of biochemical markers, lipid metabolism and blood coagulation mechanism.Results: Shenluotong can improve the MsPGN patients' main symptoms .The entirely remission rate and total effective rate of therapeutic group were 25% and 82.1%, remarkable higher than 20% and 53.3% of control group (p<0.05).The relapseing rate of therapeutic group was only 7.1%, remarkable lower than 30% of control group (p<0.05). a. Shenluotong can obviously reduce MsPGN rat's urinary protein, enhance plasma albumin, improve renal function .It can also improve glomerulus lipid metabolism and blood coagulation process. Compared with Shenyanshu group,it has evident difference (PO.05).Compared with normal contrast group, the change of biochemical target in model group has significant difference, such as urinary protein , Bun, Scr, TP, ALb and 3 2-MG and so on (P<0.05). But the shenluotong group is inferior than the model group, and has remarkable difference(P<0.01). At the same time the shenluotong group has evident difference compared with shenyanshu (P<0.05).Compared with normal contrast group,the changes of lipid metabolism and blood coagulation mechanism in model group have remarkable difference(P<0.05),such as Thromboxane B2, 6-keto-PGFia, nitricoxide synthase andendothelins and so on. But the TXB2 and ET of Shenluotong group is inferior than the model group which has evident difference(P<0.01), and the Shenluotong group has remarkable difference compared with Shenyanshu group (PO.05). The 6-keto-PGFia and NOS of shenluotong group is more than the model group which has evident difference(P<0.01), and the Shenluotong group has remarkable difference compared with Shenyanshu group (P<0.05). The TC, TG^ LDL and VLDL of model group is more than the normal contrast group (PO.05), but the HDL of model group is less than the normal contrast group(PO.05). The TC TG LDL and VLDL of shenluotong group is less than the model group (PO.Ol)and the shenyanshu group (PO.05), but the HDL of shenluotong group is more than the model group (PO.01) and the shenyanshu group (PO.05).The renal constitution pathomorphology of model group is mesangial cell proliferation , lumen of blood capillary constriction and extracellular matrix expansion etc, which has remarkable difference compared with Shenluotong group and Shenyanshu group. At the same time the Shenluotong group has evident difference compared with the Shenyanshu group.Conclusion : Shenluotong is a kind of effective recipe to treat MsPGN. Shenluotong can obviously reduce MsPGN rat's urinary protein, enhance plasma albumin,improve renal function, improve glomerulus lipid metabolism and blood coagulation process,decrease mesangial cell proliferation and extracellular matrix expansion, which means the multimode curative effect of Shenluotong may be the main mechanism that shenluotong prevents and cures MsPGN .
