Objectives and Methods: In this study, the expression of intercellular adhesion molecule-1 (1CAM-1,CD54) on the surface of lymphocytes in peripheral blood from the patients with chronic obstructive pulmonary disease(COPD)was evaluated by means of Flow Cytometric Analysis.Meantime, the concentrations of TNF- a G-CSF and sEL-2r in serum from these patients was also measured by means of ELISA. Furthermore ,the correlation between such three cytokines and CD54 was investgated. On the other hand, the relation between bacterial infection smoking and CD54 was also studied in order to find out whether intercellular adhesion molecule-1 on the surface of lymphocytes in peripheral blood was involved in the immunopathological process of COPD,and to find out the characteristic and influence factors of its expression in die patients of COPD.Results: 1.Compared with control subjects ,patients with exacerbated COPD showed an increased expression of CD54(P0.01).During stables .compared withexacerbations ,the expression of CD54 was reduced(P<0.01) ,but compared with controls ,it was still higher(P<0.01).2. Compared with control subjects , the concentrations of TNF- a N G-CSF and sIL-2r in serum from patients with exacerbated COPD were increased(P<0.01), During stables ,compared with exacerbations ,the serum level of such three cytokines was reduced(P<0.01). Compared with control subjects , the serum level of TNF- a and sIL-2r was still higher(P<0.01),but the serum level of G-CSF ruduced to abnormal level(P>0.05).3.Simple correlation analysis showed that in patients with exacerbated COPD,the serum concentrations of TNF- a and G-CSF were both positively correlated to the expression of CD54(P<0.01).But multiple correlation analysis showed that except G-CSF,only TNF- a was positively correlated to the expression of CD54(P<0.01). In patients with stable COPD,the serum concentrations of TNF- a was still positively correlated to the expression of CD54(P<0.01).And during stables ,smoking index was also positively correlated to the expression of CD54(P<0.01).4.Compared with patients without bacterial infection, patients with bacterial infection had an significantly higer expression of CD54 on the lymphocytes of peripheral blood(p<0.01).5.Both in exacerbated and stable patients of COPD,the expression of CD54 was positively correlated to the serum concentration of s!L-2r(p<0.01). Conclusions: This study suggested us that CD54expressed on the surface of lymphocytes in peripheral blood may play an important role hi immunopathological process of COPD. And, bacterial infection smoking as well as some cytokines ,for example , TNF- a may involved hi the upregulated of CD54.CD54 may involve in the immunopathological process of COPD by means of activating different kinds of lymphocytes.Anti-adhesion therapy may become a new method to control COPD in the future.
