Objective: To evaluate the effects of Monoammonium Glycyrrhetate (MAG) on the prevention of peritoneal adhesions in rats .Methods: The study included two parts.In the First part, the effects of the different concentrations of MAG on adhesion formation were observed.In the Second part,the synergistic effects in preventing adhesion formation were observed by MAG and compound injection Saloiae Miltiorrhizam (cSM).Induced standard rat models of uterine horn adhesion by using surgery gauze scraping and a scalpel scratching the serosal side of the uterine horn to make the serosal surface punctuated blooding.In the first part the rats were divided randomly into five groups with 10 rats in each. They were injected single dose intraperitoneal (ip)and orally administered(po) MAG with 25mg/kg x 5mg/kg > 1 mg/kg,Dexamethone (Dex) with 1.5mg/kg and Normal saline (NS) with lOml/kg for 6 days,respectively. The followed data were analysed : The uterine horn adhesion score > the total number of neutrophils (WBC ) in the peritoneal fluid and the thymus'weight .In the second part, the rats was divided into six groups with 10 in each. The laparotomy procedure was the same .They were injectedsingle dose ip and po MAG with of 5mg/kg > SM Iml/kg+MAG 5mg/kg, SMC lml/kg> Dex Img/kg and NS 10 ml/kg for 6 days,while the control (-) group no operation and medication . After 7 days , killed rats and determined theadhesion score ,counting the total numbers ofi neutrophils,collecting the peritoned fluid for quantification ofIL-2 , TNF- a N tPA-a and PAI-a,resecting the adhesion tissue to analyse pathological changes and tPA-a, PAI-a ,resecting the thymus to weight and analyse CD4/CD8.Results:!.In the first part,compared with NS group(9.15 +1.13),MAG (4.36 + 2.62, 4.18 + 1.5, 4.39+2.0)and Dex (2.94 + 1.55)reduced the adhesion score significantly (P<0.01) .The antiinflammation effect of MAG and Dex can be seen from WBC in the peritoneal fluid (p<0.01). Compared with other groups, Dex decreased the thymus weight (p<0.01). 2.1 Compared with NS group, cSM +MAG , MAG, Dex reduce the adhesion score (p<0.01). Obvious effect of cSM +MAG was observed compared with administrating MAG alone on prevention adhesion(P<0.05).Adminstrated SM alone was't benefical for adhesion. 2.2 The antiinflammation effect of MAG on pathology was as the same as it of Dex, while cSM has no. 2.3 Compared with NS group,MAG group and Dex group reduced WBC of peritoneal fluid, while MAG+cSM, cSM group increased WBC.(P<0.01).2.4 Compared with the others, Dex increased thymus weight (p<0.01) . 2.5 MAG groupincreased IL-2 when compared with the others' decreased level (p<0.05);Compared with control (-) group, NS group increased TNF- a level largely (p<0.05). Compared with NS group,the other group decrease TNF- a except cSM group(p<0.05).2.6 Analysed fibrinolytic system demonstrate that the injury(NS group)increase PAI-a -. decreased tPA-a and tPA/PAI ,MAG ^ cSM and Dex all alleviated the change .The results of PAI-a was significant in tissue and was insignificant in peritoneal fluid. 2.7. Both MAG +cSM and cSM increased the CD4/CD8 ratio, demonstrate that synergistic administration improve the immune(p<0.01).Conclusion: I.MAG inhibit the peritoneal adhesion in rats without significant side effect.2.The result is in agree with the opinion that the postoperative adhesion is related to the inflammation and the tissue fibrinolytic activity. 3. cSM increase CD4, CD4/CD8 and tPA, tPA/PAI.It suggest that cSM can improve the immunity and tissue fibrinolytic activity.Combined with cSM.MAG reduce PAI and play a role of prevention adhesion AThe role of MAG indicate that it can regulate the immunity by improving IL-2 and reducing TNF-a 5.The efficacy of MAG and MAG+cSM is as the same as Dex,but it isn't accompanied with any side effects of Dex.6.The efficacy of MAG+cSM is better than it of MAG alone on prevention adhesion and up-regulated fibrinolytic activity, but it required further investication on clinical application.
