Objective: In order to confirm the relationship between the level of cTnl and the myocardial injury in the patients with VMC. 54 cases were investigated. And evaluate cTnl measurements during acute viral myocarditis and assess the predictive value of cTnl measurements in acute viral myocarditis .Methods: The total of 54 patients were studied. And all cases were divided into three groups(groupA,Band C). Group A consisted of 10 VMC patients who had not any abnormal shows of ECG. Group B consisted of 37 VMC patients with one of abnormal ECG. Group C consisted of 7 VMC patients with two or more abnormal ECG. A control group consisted of 32 cases, none of them were known to have had clinical findings of VMC or any other heart diseases. Blood samples were obtained at admission, <2w, <4w, 4~8w and >3m later from 54 patients.Results: The results ,in the 2 weeks phases of VMC ,showed that total positive rats of cTnl (>3.2 ugL-1) and CK-MB(>30IUL'') in 54 patients were 70.4% (38/54) and 18.5%(10/54) respectively. There was a statistically significant increase in the positive rat of cardiac troponin I (38 vs 10, p< 0.001).The average level of serum cTnl in 54 patients with VMC (10.5 +7.63)was higher than it in control group(1.87+3.74), increased significantly (P<0.01 respectively).In the 3 months phases of VMC, 31 patients were measured .The rat of cTnl turning to be in normal range was 64.5%(20/31). The rat of symptoms improved and arrhythmia stopped were very lower significantly in the patients with higher serum cTnl than in whom had normal level of serum cTnl respectively.Conclusion: Troponin I is a sensitive and specific marker for myocardial injury and can be used to predict the likelihood of future cardiac events. It is not much more expensive to measure than CK-MB. Overall, troponin I is a better cardiac marker than CK-MB and should become the preferred cardiac enzyme when evaluating patients with suspected VMC. So the measurement of cTnl is a useful serologic test for the early diagnosis of myocarditis or myocardial cell injury and the level of serum cTnl shows very important predictive value for long-term prognosis in patients with VMC.
