The Expression And Clinical Significance Of P27～(KIP1)p57～(KIP2) In Thyroid Carcinoma

Objective: To investigate the expression pattern of p27^KIP1 and p57^KIP2 in multinodular goiter,thyroid adenoma and thyroid carcinoma, and the correlation with development as well as prognosis of thyroid carcinoma.Methods: The expression of p27^KIP1 and p57^KIP2 detected byimmunohistochemical method in 6 multinodular goiters, 10 thyroid adenomas and 35 thyroid carcinomas were analyzed to demonstrate the correlation to the clinical stages , pathological types and lymph node metastasis.Results: (1) In nucleus,the expression level of p27^KIP1and p57^KIP2 in thyroid carcinomas was significantly lower than that in multinodular goiters and thyroid adenomas (p<0.05,p<0.01 and p<0.01 ,p<0.01) . (2)In thyroid carcinoma group, the expression of p27^KIP1and p57^KIP2 in the follicular carcinoma was significantly higher than that in medullary and undiiferentiated carcinoma (p<0.01,p<0.05 and p<0.05,p<0.01 ) . In nucleus,the expression level of p27^KIP1 and p57^KIP2 protein in thyroid carcinoma group with no lymph node metastasis , no local invasion and clinical stage â… ,â…¡ was significantly higher than that with lymph node metatasis, local invasion and clinical stage â…¢,â…£. (p<0.01,p<0.05,p<0.01 and p<0.05,p<0.01,p<0.01) .(3) The positive rate of p27^KIP1and p57^KIP2 incytoplasm with lymph node metastasis was significantly higher than that with no lymph node metastasis (p<0.05,p<0.01). (4) The survival rate of 2 and 3 years after surgery in the thyroid carcinoma group with p27^KIP1expression in nucleus was significantly higher than that without p27^KIP1 expression . (5) In thyroid carcinoma the expression of p27^KIP1 appeared positive correlation with p57^KIP2(p<0.01, rs' =0.730).Conclusion: (1) The expression of p27^KIP1 and p57^KIP2 in nucleus in thyroid carcinoma has showed significant correlation with clinical stages, lymph node metastasis and local invasion .The lower expression in nucleus may associated with poor prognosis. (2) The positive expression of p27^KIP1and p57^KIP2) in cytoplasm may suggest lymph node metastasis. (3) The application of p27^KIP1 and p57^KIP2 expression pattern for pathological types is of limitation.(4) p27^KIP1 exhibits cooperation with p57^KIP2 as a inhibitor in the course of proliferation of carcinomas.
