PrefaceIncidence of ovarian cancer is the second among women with malignancies, while its death rate is the first. Because it is difficultily found early and resistant to drug, the 5 - year survival rate for all stages of ovarian cancer has remained constant at 20 -30%. Thus, it is public to focus attention whether cell biological factors affect on prognosis of ovarian cancer and biological treatment provide the new path for diagnosis and treatment of ovarian cancer. In this study , the expression of p27kipl, p53 protein was investigated by immunohisto-chemistry on tissue samples from 56 patient affected by epithelial ovarian tumors ,and the correlation with patient outcome was evaluated.Material and MethodThe study included 56 primiary ovarian cancer patients who underwent surgical resection in Chinese medical university affiated the first hospital .46 patients had serous cystadenocarcinoma, whease 10 patients had mucinous cystadenrcinoma. Patients had stage II, III, IV disease according to FIGO staging system. Histological classification of tumors was carried out according to the WHO system, and tumorswere graded as well (Gl) , moderately ( G2) , and poorly differentiated (G3). At first surgery, 39 patients had <2cm residual disease, wheares 17 patient had 5=2cm residual disease at surgery. None of the patients had recived chemotherapy or radiation therapy before surgical resection. All patients were treated with a platinum - based chemo-therapeutic regimen. Follow - up data were available for all 56 patients. ( median follow - up 21 months; rang 6-76 months). Expression of p27kipl and p53 proteins was retrospectively studied by immu-nohistochemistry the relationship between the above marks and the prognosis of the patients were then analyzed. Statistical analyses were performed using SPSS 10. 0 software. Survival curves were calculated using Kaplan - Meier method, and log - rank test used for the analysis. The prognostic significance of p27kipl and p53> proteins expression for survival was assessed in a multivariate with Cox proportional hazards model.ResultThe expression rates of p27kipl protein in 56 epithelial ovarian cancer were 41.1%. The expression rate of p53 in 56 epithelial ovarian cancer were 48.2%. there were no correlation between the expression of the both proteins ( p > 0. 05 ). There were correction between the expression of p27kipl, p53 proteins and tumor grade. In univari-ate analysis, there was significance in the media survival, the each surival rate in survival curve between the patients exhibited any p27kipl, p53 overpression and the patients exhibited no p27kipl, p53 overpression (p < 0. 05). In the multivariate analysis, only stage, residual tumor at primary surgery and p27kip 1 were the impor-tant prognostic factors of the patients with EOC, and there was no significant correlation between the overpression of p53 protein with the prognosis. Conclusions: The expression of p27kipl was an independent prognostic factor for prognosis of the patients with EOC. There was no significant correlation between the overexpression of p53 protein and the prognosis of the patients with EOC.DiscuseThe p27kipl gene is a member of the cip/kip family of cyclin -dependent kinase inhibitors ( CKIS). p27kipl is a CKIS that regulates progression from Gl into S phase by inhibiting a variety of cyclin - cdk complexes and it plays an important role in regulating cell proliferation, differenciation and apotosis. Many studies show that p27kipl gene mutant is rare and the expression level and activity are related to tumorigenesis and prognosis. Several studies have demonstrated that loss of p27kipl protein is a negative prognostic marker in breast, colon, lung, prostate cancer.This study found that the media suvival of patients with p27kipl positive expression was longer than that of patients with p27kipl no expression.
