Objective: The aim of this research is to evaluate the value of cardiac troponin I (cTnI) in infants with congenital heart disease (CHD) peri-open heart surgery.Methods: The cTnI, CK, CK-MB, LDH and AST were measured respectively before operation, at the end and at 24h, 48h, 72h after surgery in 33 cases with CHD (6 cases of VSD, 9 cases of VSD+PH, 3 cases of ASD, 4 cases of VSD+ASD, 5 cases of TOP, 2 cases of TGA and one case of POF, TA, PA, PS) respectively.Results: cTnl reach the highest level at the end of operation and maintain a high level at 24h, 48h after surgery. At 72h after surgery, the cTnl is normal. Compare low cardiac output group (n=14) with no low cardiac output group (n=19), there were distinct difference (p<0.01~0.05) at the end of operation and at 24h after surgery. In 3 died patients, the cTnl elevate dramatically (>100ng/ml) after surgery. But the level of CK,CK-MB at 24h after operation were higher than before surgery (p<0.01). LDH, AST were maintain a high level from 48h to 72h after surgery. Comparing with CK, CK-MB, LDH and AST, the cTnI raise early, obviously and long.Conclusion: The cTnl can reflect the damage of myocardium peri-open heart surgery. It is more sensitive than CK, CK-MB, LDH and AST. The cTnI increase sharply (>100ng/ml) indicate severe myocardium damage and poor prognosis.
