AIM : Primary carcinoma of liver is NO.4 death causation of human being around the world. It is one of the commonest malignant tumours in our country. Hepatocellular carcinoma is a well-known tumor for its higher invasive and metastasis activity and poor prognosis. The research of mechanism of aggression and metastasis of hepatocellular carcinoma had become emphases of the preventing and treating of HCC. It is reported during the invasion of tumor CD facilitate invasion of tumor cell by hydrolyzing. The basic fibroblast growth factor (b-FGF)play a critical role in the process of development, invasion, metastasis in many kinds of tumor. Furthermore, it can be used to judge malignant degree and predict prognosis. The goal of this experiment is to research the mechanism of excess expression of CD in hepatocellular carcinoma and mechanism of aggression and metastasis of hepatocellular carcinoma by research the expression of cathepsin D and basic fibroblast growth factor in hepatocellular carcinoma. I want to search a new technique to clinic preventing and treating metastasis of hepatocellular carcinoma. Material and MethodTissues of HCC were obtained from 42 patients who underwent surgery. There were 32 males and 10 females. The age of patients ranged from 30 to 65. The mean age is 47.5. 33 patients AFP level exceeded 400ng/ml(78.6%), 32 patients' HBsAg.was positive (76.2%).The diameter of tumor(d  5cm,8 samples; 5 cm10cm,9 samples)ranged from 3cm to 15 cm(mean:7.1cm). According to the criteria describe by Edmondson and Steiner. Tumour were graded to( grade 1 , 5 patients, Grade 2, 14 cases, Grade 3, 21 cases, Grade 4 , 2 patients). 17 cases were well encapsulated and 25 cases were poor encapsulated. 13 cases had thrombosis and 29 cases had not. No patients had received radiotherapy, chemotherapy, trans arterial embolization or immunotherapy before the surgery. Five cases with cyst of liver were used as normal controls. Anti CD and Anti b-FGF monoclonal antibodies were used to perform immunhistochemistry and the specimens were stained with SP method. The frame of cell is clear and there were brown grained in the cell and the paint was more evident than the background is CD positive.There are brown grains in the cell and the paint was more evident than the background is b-FGF positive. Statistical analysis was executed by SPSS 10.0 software. a values less than 0.05 was considered significant.Result:1 . The expression of CD is positive in normal controls. But the expression cell less than 20%.The expression of b-FGF is negative in normal controls.2. The expression of CD and b-FGF have no relationship with the sex of patients and the levels of AFP before surgery and positive or negative of HBsAg.(P>0.05)3. There were no obvious relationships between CD and diameter of tumour. There are 6 cases positive with the diameter less than 5cm.(75.0%).22 cases positive with the diameter ranged from 5cm to 10cm.(88.0%), There are 8 cases positive with the diameter more than 10cm.(88.9%)There were more common in grade 3-4 (22/23 95.7%)than in grade 1-2(14/19 73.7%)tumors. The positive rate of the group which were well encapsulated is 82.4%(14/17)and 88.0%(22/25)in the group without intact membrane or without membrane. The positive rates in the group with thrombosis is 84.6%(11/13)and 86.2%(25/29) in the group without thrombosis. There is not significant difference between these two groups.4. The expression rate of b-FGF is 62.5%(5/8) with the diameter less than 5cm and64.0% with the diameter ranged from 5cm to 10cm , 88.7% with the diameter more than lOcm.The expression rate of b-FGF is 42.1%(8/19) in grade 1-2. And the expression rate of b-FGF is 91.3%(21/23) in grade 3-4.The positive rate of the group which were well encapsulated is 41.2%(7/17)and 88.0%(22/25)in the group without intact membrane or without membrane.
