Purpose To study on the relationship between the expression feature of basic fibroblast growth factor(bFGF) and proliferative state in the breast cancer, and looking for a biological marks for diagnosis of the breast carcinoma.Methods 44 cases of breast carcinoma specimens and 33 cases of fibroadenoma and 30 cases of adenosis (control group) were fixed and embedded by routine pathologic technique and serial sections (4 μ m). Immunostains of bFGF (1:200), VEGF(1:1000), p53(1:150), Ki67(1:80), cyclinD1(1:225) and FVIII-RA(1:50), CD31(1:30) and CD34(1:100) were conducted by ABC method. The microvessel density(MVD) was estimated with Weidner and Maeda Method.Results:1. The bFGF positive products were on the membrane of the cells in the breast carcinoma, a few in the cytoplasm, and negative for nuclei,but mainly in the cytoplasm and the nuclei in the ductal epithelial hyperplasia of the fibroadenoma and adenosis.2. Positive rates of bFGF on the membrane of the cells in the breast carcinoma was 63.64%(28/44). Microvessel density(MVD) of which was 86.05?2.49 and higher than negative of bFGF on the membrane of the cells in the breast carcinoma MVD54.9± 10.81, (P0.01). Positive of bFGF on the membrane of the cells in the breast carcinoma was correlated with VEGF expression, cyclinD1 overexpression, Ki-67 high proliferative index(>12%)(P<0.05),and not related to p53 protein(P=0.054).3. VEGF positive products , in the breast carcinoma, fibroadenoma and adenosis were mainly in the cytoplasm of the cells of carcinoma cells, myoepithelial, hyperplasia ductal epithelial and endoepithelial of vessel. Positive rates of VEGF in the cytoplasm of the cells of carcinoma cells was 84.09%(37/44) and greater than positive rates of VEGF in fibroadenoma 51.52%(17/44)and adenosis 60%(18/44)( P<0.01). The positive rates ofVEGF in the breast carcinoma was correlated with MVD . MVD 84.83 ±22.09 of the positive expression of VEGF was significently higher than MVD 47.9 ±2.61 in the breast carcinoma negative to VEGF (P0.01).4. The positive products of P53 > CyclinDl and Ki67 were all located in the cell nuclei. p53 positive products 54.55%(24/44) of which was not correlated with MVD(P=0.983), were only deposited in the carcinoma cells (P=0.591). CyclinD1-overexpression was 56.8%(25/44) in breast carcinoma and greater than that in fibroadenoma 6.06%(2/33)and adenosis 10%(3/30)(P<0.01). Ki67 high proliferative index was occurred with 63.64%(28/44) in carcinoma cells and greater than that with 9.01%(3/33) in fibroadenoma and adenosis 0%(0/30)(P<0.01).5. The endoepithelial cells of vessels were marked by CD34 which was a better one of the immunohistochemical reseach for tumor angiogenesis. MVD of 44 cases of breast carcinoma was 80. 21 ±24. 07. The MVD was increased to 92.88± 19.33,when the diameter of 12cases out of 44 cases of carcinoma was over 4cm. MVD in fibroadenoma was 48.33± 17.44and adenosis 45.28±14.04 were very significant to compare with the 80. 21±24.07 MVD in breast carcinoma(P<0.01).Conclusions1. The positive products of bFGF immunostain in breast carcinoma was mainly on the carcinoma cell membrane was correlated positively with VEGF expression, cyclinDl overexpression, Ki-67 high proliferative index and MVD, and suggested that the cells of the membrane positive products were situated in malignant proliferative state.2. The located features of bFGF expression combined with immunostain for the p53, cyclinDl and Ki67 may be useful to predict the tendency of malignant transformation of benign hyperplasia lesions or to diagnosis of breast carcinoma.
