In present, Owing to the amount of operation on aged patients over 65 years increasing gradually, the dangerous factors relating to age during perioperation, have been on the increase accordingly. Among these factors, the commonest is hypertension, especially during induction, becase stress is so intensive that hemodynamics is unstable. The temporary high blood pressure and heart rate is no danger to health people, but for elderly hypertensive patients, it can result in cardiovascular risk events. With the development of biochemistry and radioimmunoassay(RIA) in recent years, the function of neuropeptides in circulatory has been valued. Neuropeptide Y (NPY) has vaso-constricting effect, whereas calcitonin gene-related peptide(CGRP) has strong vaso-dilating and positive inotropic action. But the effect of intravenous anesthetics on neuropeptides is less mentioned. So the objective of this experiment is to assess the effects of the different intravenous anesthetics on the plasma levels of two neuropeptides in elderly hypertensive patients undergoing upper abdominal surgery during induction in order to find the best intravenous induction agent for the hypertension and provide the theoretical warranty for clinical anesthesia.Materials and Methods: thirty elderly hypertensive patients (SBP>140mmHg, DBP>90mmHg, WHO/ISH, 1999) including 17 men and 13 women with a mean age of (70 5) yr were randomly divided into 3 equal groups(10 each): thiopental group (A), etomidate group (B) and propofol group (C). All 30 patients underwent abdominal surgery. The function of cardiovascular, hepar, lung and kidney wasnt obviouslyabnormal, ASA I ~ II . In addition, selecting ten normal blood pressure elderly people as control group (D). venous blood samples were taken befor anesthesia (Ti),2min after induction(T2) and 2 and 10 min after tracheal intubation(T3, Tj ) for determination of plasma levels of NPY and CGRP measured with radioimmoassay(RIA). In the same time, SBP, DBP and HR were recorded as above.Results: The changes of hemodynamic: (1)Group A, B, C were compared with group D before anesthesia. The content of SBP, DBP and HR were higher than that in group D respectively. (SP: 156.2 10.7mmhg, 160.8 11.6mmHg, 158.7 8.7mmHg, 122.2 13.4mmHg ; DP : 95.4 5.4mmHg, 97.3 5.2mmHg,99.9 5.5mmHg, 75.4 9.3mmHg; HR: 83.60 15.1bpm, 85.60 14.3, 87.2 14.3bpm, 74.3 11.7bpm, P<0.01), and no statistically significient differences were identified among group A, B and C. (2)SBP and DBP significantly decreased after induction in group A and group C( SP: 137.3 13.5mmHg, 158.0 9.5mmHg;DP: 87.5 6.3mmHg, 80.2 9.6mmHg, PO.01), also the contents in group C were lower than mat in group A(P<0.05).; HR is increased significiently in group A after induction (91.00 22.5bpm, PO.05), while group B and group C(86.20 12.7bpm, 80.80 21.0bpm) were similar with TI. (3)SBP, DBP and HR significantly increased after intuction in group A and group C(SP: 174.5 16.9mmHg, 177.9 14.9mmHg; DP: 106.2 8.4mmHg, 108.4 9.0mmHg; HR: 113.40 24.2 , 117.10 24.3, P<0.01); there was no chang in group C ( 159.0 9.7 mmHg, 96.6 9.8 mmHg, 89.3 2.0bpm) and lower than that in group A and group B. (D No statistically significient differences were identified among group A, B and C at T4(SP: 1 55. 1 7.6 mmHg, 159.6 7.7mmHg,157.3 7.9 mmHg; DP: 97.6 7.4 mmHg, 95.9 6.7mmHg,99.4 8.1mmHg; HR: 85.2 20.4bpm, 87.5 18.52bpm, 89.4 12.28bpm) compared with T1 .The changes of NPY and CGRP: (1)Group A, B, C were compared with group D before anesthesia. The content of NPY were higher than that in group D respectively., (125.78 17.46pg/ml, 123.88 14.78pg/ml, 121.75 18.93pg/ml, 91.75 18.31pg/ml , P<0.01); but the content of CGRP in Group A, B and C was lower in group D respectively..(65.86 9.38pg/ml,63.59 8.50pg/ml,61 .36 8.17pg/ml,75.50 l 1 . 1 5pg/ml , PO.01); (2)NPY significantly decreased after induction in group A and group C (106.80 14.75pg/ml, 103.94 11.62pg/ml, PO.01), while CGRP significantly increasedafter induction in group C (75.99 13.87pg...
