Objective :Antiepileptic drugs (AEDS) may affect serum thyroid hormone concentration. this study aimed to evaluate thyroid function in men taking carbamazepine(CBZ) ,or valproate(VPA) for epilepsy.Methods: Serum thyroid hormone ,gamma-glutamyl-transferase (GOT) and antithyroid antibody (ab) concentrations in 50 epileptic patients(25 taking CBZ,25 taking VPA monotherapy) were measured before treatment with antiepileptic drugs and compared it with 30 healthy controls.The changes of thyroid hormone levels were observed after 6 months treated with carbamazepine(CBZ) or sodium valproate(VPA) and the correlation between thyroid hormone and GGT or antithyroid antibody were studied. Results : (1) There was no significant difference in serum thyroid hormone ,GGT and antithyroid antibody between the patients without treatment and the controls(P>0.05). (2) In men taking CBZ: (1) Serum thyroxine (T4) , free thyroxine (FT4), rT3,Ts were significantly lower after treatment of CBZ in 6 months later(P<0.05).twelve percent of men taking CBZ had serum T4 and 48% had serum FT4,52% had serum rT3,32% had serum T3 levels below the reference range . (2) Serum free triiodothyonine(FT3) and thyrotropin(TSH) ,GGT ,antibody in men taking CBZ were normal(P>0.05) (3) There was no significant correlations between changes of thyroid hormones and the concentrations of GGT(P>0.05).(4)There was no significant correlations between changes of thyroid hormones and antithyroid antibody was found(P>0.05). (3) In men taking VPA ,the concentrations of thyroid hormones, thyrotropin ,GGT, and antithyroid antibody were normal(P>0.05).Conclusion : 1 epilepsy itself do not affects thyroid function ,2 Serum thyroid hormone concentrations are low in CBZ treated men ,however,these low levels do not seem to be due to liver enzyme induction or activation of autoimmunologic mechanisms.Therefore ,CBZ may have effects on the function of the hypothalamo-pituitary axis.3 VPA does not have significant effects on thyroid function.
