Objective:The aim of this study was to investigate microvessel density(MVD) and expression of vascular endothelial growth factor(VEGF),androgen receptor(AR) in the prostates of men who received reoperation after TURP and to evaluate their clinical significance.Methods:The study included 100 cases underwent TURP for BPH of whom 50 were performed re-TURP(re-TURP group) and the remaining 50 served as controls.150 specimens were collected in all. Sections were stained for CD34 specific for nascent blood vessels and VEGF,AR by immunohistochemistry(S-P).Statistical analysis of the results was performed using t-test.Results:(1) MVD parameter was 35.83±20.92 in re-TURP group and 20.56±6.99 in control group,statistic significance existed between the two groups(P<0.05);MVD parameter was 32.16±16.65 in the second TURP specimens,difference in MVD were not found to be significantly different between the first and the second TURP specimens(P>0.05).(2)The expression of VEGF was significantly higher in the re-TURP group(VEGF index:7.55±2.72) compared to controls(VEGF index:4.28±2.62),while there was no statistical significance between the first and the second TURP specimens(P>0.05).(3)The expression of AR in the first and second TURP specimens as well as controls were 6.17±1.86,6.99±2.44 and 4.16±1.34 respectively.Statistic significance existed between the first TURP specimens and control groups,although the second TURP specimens showed higher AR index than the first TURP specimens,the difference was not statistically significant(P>0.05).(4) Different correlations were observed among the MVD,VEGF and AR in all groups:MVD positively correlated with VEGF(r=0.705,P<0.05)and AR(r=0.525,P<0.05); VEGF positively correlated with AR(r=0.650,P<0.05).Conclusions:MVD and VEGF was activated abnormally in BPH, especially in re-TURP groups.The expression of AR and VEGF correlated with angiogenesis in BPH.AR and VEGF may alter the biological behavior of BPH by prompting angiogenesis.AR may positively adjust high VEGF expression in BPH.Increased expression of VEGF and AR as well as higher MVD in prostatic tissue may play an important role in the pathological process of BPH after TURP.They may be a new target for curing of BPH and serve as key for researching mechanism of BHP.
