ObjectiveBronchial asthma is a kind of chronic airway inflammation which is caused by eosinophil, mast cell, lymphocytes and various cytokines. It is defined by three characteristic feature: intermittent reversible airway obstruction, airway hy-perresponsiveness ( AHR) , and airway inflammation. To asthma , earlier period for diagnosis and cure have great important significance through judgement the existence and the severity of airway inflammation. We usually judgement the airway inflammation indirectly according to clinical symptom, measurement of pulmonary function, treatment result and AHR, or evaluation the condition of the airway inflammation according to bronchoalveolar lavage and bronchi lung biopsy. The former often cant really reflect the condition of the airway inflammation of asthma patients and the tolerance and compliance of patients to bronchoalveolar lavage and endobronchial biopsies is very low, so its application is limited. For this reason,at present,internal and external scholars are exploring a detect method which is non - invasive, specificity, easy to grasp and some specific markers for airway inflammation. In this study, induced sputum samples analyzed for eosinophilic cationic protein and granulocyte - macrophage colony stimulating factor before and after glucocorticoid treatment and the correlation between them and the degree of airway obstruction, and discuss the action of inflammation markers in induced sputum in asthma.MethodsTwenty subjects (9 females and 11 males) with mild, moderate asthma, aged 18 to 64 years, participated in the study. The forced expiratory volume in one second (FEV, ) was poor to 80% of predicted and amelioration rate increaseto 15%. All subjectes inhaled glucocorticoid for four weeks. Pre - and post -treatment we all process the induced sputum and pulmonary function measurement.Sputum induction: For 15 minutes after received bronchi dilataltion ( Vento-lin)200fjLg, FEV\was measured for all subjects. If was increased to 60% of predicted , then we inducing sputum by an ultrasonic nebulizer. The concentration of sodium chloride is 3%. Weighing the thick portion of sputum sample and four times volumes of dithiothreitol 0. 1% was added. The samples were gently mixed,and placed in a shaking water bath at 37Tl for 15 minutes to ensure complete homogenization. After filtered through a nylon gauze, cytocentrifuged for 5 minutes at 1000rpm. The supernatant was aspirated and store at -70X1.Biochemical assays: In sputum supernatant, ECP was assessed by fluoro-enzyme immuno assay, and GM - CSF by enzyme - linked immunosorbent assay.Analysis: Use Student' s t - test for paired data and linear regression analysis for statistics disposal.Results1. As expected, ECP, GM - CSF and FEV, ( mean sD) differed significantly between pre - and post - treatment (ECP: 17. 42 17. 99 g/L vs 4. 45 5.33 g/L, p=0.001;GM-CSF: 45.73 12.59pg/ml vs 29.44 6.36Pg/ ml, p<0.01; FEV1:2.06 0.67L/s vs 2. 39 0. 65L/s,p <0.01).2. It is differed significantly between mild and moderate asthma in ECP,GM -CSFandFEV1(p<0.01).3. The level of ECP in induced sputum take on a significant negative correlation with FEV1 (r = - 0. 707, p = 0. 001) ; the level of GM - CSF in induced sputum take on a significant negative correlation with FEV1 ( r = - 0. 586, p = 0. 007 ) ; the level of ECP take on a significant positive correlation with GM - CSF (r=0.694,p=0.001).DiscussionAsthma has chronic airway inflammation, it is very important to assess the state of illness through the measurement of cytokines and inflammation markers from inflammation local constitution. Sputum induction has the character of non - invasive, plenty sources of the sample, easy to obtain and satisfactorily repro-ducibility. It has high coincidence rate with BAL and bronchi lung biopsy. Induced sputum is the secretion of peri - airway to center - airway, it has more intensive cell and biochemical component, which can really reflect the concentration of airway secretion at physio - behavior.
