IntroductionFollowing the older persons are more and more, the dangerous of congestive heart failure to the men's health, specially to the older men, are more and more bigger. During the studying the congestive heart failure, people pay attention to the action of insulin resistance more and more. The cardiology results suggested there were insulinresistance, hyperleptinaemia and leptin resistance in heart failure. Huperleptinaemia is only that can lonely propect insulin resistance.Objects And MethodsStudy population:According to WHO (1995) diagnosis criteria, we selected 30 patients with moderate to serious congestive heart failure who have no diabetes, bad action of liver and kidney that could affect patients'insulin level. Their age are form 36 to 84 years. Etiology of congestive heart failure inclused coronary atherosclerosis, hupertension, rheumatic heart disease or dilated cardiomypathy etc. We also selected 10 healthy men for the control. Their age are form 35 to 75 years.Measurements of blood samplesAfter at least 12 hours of fasting, blood samples were collected after patients had been lying undisturbed in a supire position for at least 10 minutes. Blood was drawn by venipuncture. After been put straightly for 1 hour the samples were sparated by centrifugation at 1000rpm for 15 minutes. After that the serum was immediately drawn about 1.5-2.0 ml, then frozen and stored at - 30 C until assayed for fibrinolytic components. We used in vitro sandwich Enzyme-linked Immunosorbent Assay for the quantitative measurement of human leptin in serum. The results are expressed in ug/1.Statistical analysisData are expressed as the mean sd. Comparisons among the 2 groups (congestive heart failure subjects, control subjects) were performed by analysis of variance followed by student test. Statistical evaluation was also performed by analysis of variance with repeated measurement which included the effects of insulin, C peptide and leptin and comparisons between groups and, if any changes were found to be significant by analysis of variance a paired t test was performed to the relevant data pair. Probability values of <0.05 were considered significant.ResultsBaseline characteristicsTable I shows baseline clinical characteristics of the congestive heart failure patients and the control subjects. There were no significant differences in age, body mass index, sex among the 2 groups. But there significant difference in serum leptin level among the two groups.In all the subjects, we compared the serum leptin levels of male in congestive heart failure patients with that in control subjects. They are 8.86 2.89ug/l vs 5.23 1.54ug/l. There are significant difference in serum leptin level of the male among the 2 groups (p<0.05) , the same as the female's. But there were no significant difference in leptin level of the male and the female in the same group (p>0.05) .There were significant difference in C peptide level among the congestive heart failure and the control groups (p<0.05) .But there were no significant difference in insulin level (p>0.05) .It suggested in linear correlation analysis that leptin level positive correlated with BMI in 2 groups. (r=0.347, p<0.05 )and the fasting C peptide, insulin level in congestive heart failure groups (r=0.510, 0.316, p<0.05) .DiscussionCongestive heart failure is major disease which are dangerous to people's health, specially to the older's. It suggested that there were insulin resistance (IR) , huperleptinaemia, leptin resistance in heart failure. IR was not only a factor of cardiovascular disease progression, but also a complication of heart failure. Indifferent experimental and clinical evidence suggested that huperleptinaemia was only the factor which could predict IR.As a endocrinal hormone, leptin can affect feeding thermogenesis, metabolism, reproduction and neuroendocrine status et al.
