Objective To observe the changes of chemerin,leptin,adiponectin levels in chronic heart failure(CHF)patients living in hypoxic regions and investigate their effect on ventricular remodeling.Methods From May 2013 to December 2015,we collected 285 cases of healthy controls and CHF patients diagnosed by the New York Heart Association(NYHA)criteria admitted to our department.According to the altitude they resided,they were divided into 68 cases in CHF patient group and 75 cases in the control group in the 2260 meters,and 70 cases in CHF patient group and 72 cases in the control group in the 3300 meters.The levels of serum chemerin,leptin,and adiponectin were measured,and the mean wall stress(MWS)and left ventricular mass index(LVMI)were calculated.And the corresponding statistical method is used to process the obtained data.Results 1.There are no statistically significant differences in age,sex,heart function in the four groups(p>0.05),and the difference of oxygen saturation was statistically significant(p<0.05);2.The levels of chemerin,leptin,adiponectin,MWS and LVMI in patients with CHF at elevation of 3300 m are higher than those of CHF at 2260 m(p<0.05);3.The chemerin,leptin,adiponectin levels,MWS and LVMI of the 3300 m health control group were higher than the 2260 m health control group(p<0.05);4.The levels of chemerin,leptin,adiponectin,LVMI and MWS in patients with CHF at elevation of 3300 m and 2260 m were higher than those in control groups at(p<0.05);5.The levels of MWS and LVMI in patients with CHF were positively correlated with chemerin,leptin and adiponectin respectively at elevation of 2260 m and 3300m(p<0.05).Conclusions 1.The chemerin,leptin,adiponectin levels,LVMI and MWS of patients with CHF and health control group at high altitude are higher than those in low elevation CHF group and healthy control group;The levels of chemerin,leptin,adiponectin,LVMI and MWS of the CHF patients at the same altitude are higher than their health control groups.It is suggested that the secretion of adipokine increased,the secretion of inflammatory cytokines increased,and the neuroendocrine was excessively activated in patients with CHF,and the LVMI and MWS increased markedly.2.MWS and LVMI are positively correlated with chemerin,leptin and adiponectin levels in patients with CHF.The results show that the change of adipokine level may be involved in the development of CHF in hypoxic environment.
