Being regarded as an ideal method for lung disease of the end stage, lung transplantation is one of the important parts of organ transplantation. Chronic and acute rejection has perplexed lung transplantation for a long time as a difficult question. So it has became an important thing that the choice of anti-rejection drugs.The Chinese herb tripterygium wilfordii Hook F (Tn), produced in the south of China, has the ability to control the rejection. And was to be thought highly of as an ideal drug. Studies have demonstrated that TII may inhibit the episode of rejection. But the effect of total glycosides in tripterygium wilfordii (Tn) on lung transplantation rejection has not been reported.We established the model of canine lung transplantation; the TII and CsA were given as anti-rejection drugs, and the efficacy of the TII to control the canine's immune rejection was observed. Experiment 1: The effect of total glycosides in tripterygium wilfordii (TII) onpathological change in the model of canine's graftObjective: To simulate the step and process in clinical lung transplantation, create the canine lung transplantation model, investigate the pathological change and the effect of TII in the model of canine's graft after transplantation. Methods: (1) Forty male canines were randomly divided into 4 groups . Group A: The canines were allotransplantated and the saline was fed; Group B: The canines were allotransplantated and the CsA was fed; Group C: The canines were allotransplantated and the TII was fed; Group D: the canines were allotransplantated and the TII with small dose CsA were fed.(2) the lung of canines were transplantated from donor to receptor .(3) The TII and CsA were given as anti-rejection drugs. Observed how long the graft could survive after transplantation. The pathological changes in the lung grafts after lung transplantation were examined by light and electronic microscope. Results: The survive time ofgroup A was shorter than the other groups, having the severity rejection. The pathological changes of grafts: A Large amount of inflammatory cells were seen in the alveolar cavities; mononuclear cells and lymphocytes were found around the micro-vessel in group B and group C; coagulum were found in some vessels. In the electronic microscope, some epithelial cells degeneration and necrosis occurred. Mononuclear lymphocytes and macrophages lay in the alveolar cavities. The collagen had been detected in some grafts survived for a long time in group D.Conclusions: TII combined with small dose CsA could inhibit the episode of acute rejection and to extend the survive time of grafts after lungtransplantation in canine.Experiment 2: The effect of total glycosides in tripteryghun wilfordii (TII) onexpression of TNF-a IL-2 IL-6 in the peripheral blood serum of model of canineObjective: Based on the successed creating the lung transplantation model of canine, the expression of TNF-a IL-2, IL-6 in serum were checked in different periods to investigate the effect of TII in the model of canine's serum after transplantation. Methods: Forty male canines were randomly divided into 4 groups as experiment 1.The peripheral blood of canine were collect at the first day before operation and 3 5 7 10 days after transplantation . The samples were stored in icebox at -70C after centrifugation with 5400 rpm. The expression of TNF-a IL-2, IL-6 in serum were detected with the Test Kit which provided by the department of immunity. Results: (1)The level of serum TNF-a was significantly higher in post operation than pre-operation (p<0.05) .There was no significant difference in TNF-a level among the three groups (p>0.1). (2)The level of IL-2 IL-6 in control group was higher than that in TII combined with CsA and TII alone at the same time after transplantation (p<0.05).The change of IL-6 expression in group C was very small (p>0.1).
