BACKGROUNDS: Adhesion molecules are divided into five groups: Selection, integrin, immunogolbulin superfamily, and cadherin. A large part of adhesion molecules exist on the surface of all kinds of cells. They have important effect on biology actions. Adhesion molecules are involved in cell-cell interaction and therefore probably play a role in the differentiation and egress of cell from bone marrow, which might be potentially important in the biology actions of acute promylocytic leukemia(APL). ICAM-1 mediated leukemia cell on the cell surface, it is possible that cellular ICAM-1 has an important role in disease prognosis. ICAM-1 mediated leukemia cell on the cell surface. It is reported sCD44 levels may be a useful marker for monitor response to treatment and disease prognosis especially in acute leukemia. Interaction of intercellular adhesion molecules (ICAM) with their receptor has a key role in normal leucocyte adhesion and migration, wheras in APL this has not been well established. In this study, we have evaluated the role of serum adhesion molecule ICAM-1 and CD44. Adhesion molecules have been reported in several acute myeloid leukemia (AML), The success of all-trams retinoic acid (ATRA) in the therapy of APL has received increased attention.APL is a special type of acute non-lymphoid leukemia, which has special genetic characteristics: t (15;17) translocation PML-RARα fasion gene and cancer protein PML-RARα.90 percent of cases of APL have genetic characteristics: t (15;17) translocation , PML-RARα fusion gene and cancer protein PML-RARα. After ATRA, Arsenic trioxide (As2O3) is another effective drug which is generally accepted by are world. Acute promyelocytic leukemia should be distinguished from other subtypes of acute myeloid leukemia (AML) because of the increased risk of disseminated intravascular coagulation (DIC) and its response to arsenic compounds and retinoids and leukocytosis. Arsenic trioxide (As2O3) is used in APL therapy for the high complete remission rate, high long-time survival rate, low relapse rate, light bad response, low drag-resistance rate and no Cross-resistance with all trans-retinoic acid (ATRA) and other chemotherapy medicine. As2O3 is fit for different types and periods of APL, it is permitted by FDA for clinical use. At present, ATRA and As2O3 is the best choice for APL therapy in the clinical treatment. In order to analyze the clinical Meanings of sICAM-1 and sCD44 in APL during the treatment with ATRA and As2O3, the following studies are applied. OBJECTIVE: Detecting serum levels of soluble adhesion molecules in patients could help to improve disease diagnosis ,curative effect and prognosis analysis. Investion serum levels and clinical significance of sICAM-1 and sCD44 before and after the treatment with all-trans retinoic acid (ATRA) and As2O3 for acute promyelocytic leukemia (APL) patients. MATERIALS AND METHODS: Sera of eight cases with APL (including untreated seven cases and relapse one case) and eight healthy individuals as control were collected and analyzed for concentrations sICAM-1 and sCD44 with enzyme-linked immunosobent assay (ELISA) method in the course of treatment with ATRA and As2O3 for inducing cells differentiation. RESULTS:Through the experiments we have gained several results as followings: (1) Sera levels of sICAM-1 and sCD44 were significantly higher than before treatment with ATRA and As2O3 in APL group.(2) sera levels of sICAM-1 and sCD44 were gradually declined after treatment with ATRA than before (P<0.01),and sera levels of sICAM-1 and sCD44 concentrations decreased significantly in response to the bone marrow remission (BMR) than after(P<0.01),and reached slightly higher levels than control group (P<0.05). (4) Before treatment, there was linear correlation between concentrations of sICAM-1 and sCD44 and the numbers of peripheral leukocyte cell and blast cell in bone marrow in APL.
